What the inspector wrote

Licensing Program Analyst (LPA) Ashley Calderon conducted an unannounced case management visit and met with Head Teacher Vanessa Ramos. LPA disclosed the purpose of today's visit. There was an incident that was reported to Licensing Department on April 13th, 2026 on the day the incident, the facility met reporting requirements in a timely manner. Per LIC624 written incident report submitted; Child #1 (C1) self tripped as they were instructed to line up during music and movement, C1 ran causing them to trip. Facility provided C1 with first aid care. LPA Conducted a tour of the facility alongside with Head Teacher V.Ramos. Currently the facility has 2 classrooms but only 1 is open, during today's visit LPA observed 13 children with 3 teachers on site in room #1. At approx 10:30am t he Site Manager Monica Alvarado arrived at the above location to join in. LPA observed the room where incident occurred, LPA did not observe any tripping hazards or risk to children. During this visit LPA observed C1 they were outside playing with friends in the grass area, running and participating in outdoor play with their classmates. LPA Calderon during today's visit interviewed Head Teacher V.Ramos (Staff #1), Site Manager M.Alvarado, Staff #3 witness and C1. There were 2 witnesses who observed the fall, Sub Teacher Staff #2 was not present to be interviewed during today's visit. In addition, LPA Calderon collected doctor's note for Child #1. Per Doctor's note C1 was able to return the next day after the incident on 4/14/26 and per record review no restrictions were given for C1. Per Interview with V.Ramos stated C1 parent did not bring them back until 4/15/26 as they preferred for C1 to be home for observation. Per V.Ramos stated C1 since returned has been themselves. LPA reviewed first aid kit t hat is in a emergency backpack that stays with the class when inside or outside, LPA did not observe tweezer therefore consultation was provided. No deficiencies were given. An exit interview was conducted, and a copy of this report was reviewed and provided wit Site Manager Monica Alvarado. A notice of site visit was given to post for 30 days.
