What the inspector wrote

Licensing Program Analyst (LPA), Bardo Baluyot conducted an unannounced required annual Inspection. Upon arrival, LPA met with Director Shabana Syed. The Pre School program operates on the grounds of Islamic Center of San Gabriel Valley. The Center is primarily used as a religious facility and also operates primary school (K-8) on both floors. The classrooms and director's office are located on the second floor at the rear of the building and has a separate entrance. Office Manager, Asad Saqib guided the LPA on a tour of both indoor and outdoor areas so Director Syed could remain with the children . Per Director, hours are 8 AM - 3:30 PM, Monday to Friday. Telephone service is available in the Director's office. An adult restroom which is adjacent to the classrooms and Director's office is also used by children and is equipped with a child size sink and two child size toilets. A second adult restroom located on the same floor will be used for ill/isolated children. Both classrooms were observed to be well ventilated and well equipped with plenty of natural lighting. Only Classroom 1 was in use due to current enrollment. LPA observed 7 children present and Classroom 2 to be kept lock while not in use. Per Director, napping equipment is washed weekly or as needed by staff. Carpet and flooring were observed to be clean . Carpet was installed shortly prior to licensure. LPA observed a water cooler with cups readily available for children in the classroom. Per Director, children also have personal water bottles which are brought out with them during outdoor activities and can be refilled from the water cooler when needed. LPA observed personal water bottles being kept in each child's individual cubby. First Aid Kit was located at the teacher's/director's desk in the classroom and was inventoried for quantity and expiration dates. LPA observed an Emergency Backpack also located in the main hallway. Per Mr.Saqib, all cleaning solutions are kept locked in the maintenance room on the first floor. LPA observed toys and other learning materials available for children. The ill/isolation area will be in the administrator's office and a cot was observed to be available for children to use. 1 OF 3 Refrigerator and microwave located in the Director's office were observed to be clean . Per Director, refrigerator and microwave are for staff use only. Per Director, all meals and snacks are provided by parents and don't require refrigeration. Snack menus were posted but have not but in use since the start of the pandemic. Prior to the pandemic, facility provided snacks. Licensing as well as COVID posting requirements were observed throughout the facility. LPA observed a fire extinguisher located in preschool's main hallway with a service date of 10/23/2020. Per Office Manager, there is also a pull alarm system and sprinkler system in place throughout the entire facility and the carbon monoxide detector is part of the hardwired fire system installed. Per Director, parents sign children in and out in the parking lot as parents have not been allowed into the building since the start of the pandemic. Per Director, there are no Incidental Medical Services \342\200\223 IMS at this time. IMS plan will be submitted. It is currently under a waiver to use a staggered schedule (once enrollment requires it) as it was measured to adequately accommodate only 16 children. The facility is licensed for 30 children. However, upon arrival at approximately 9:30 AM, LPA observed Director Syed outside with the children using the primary school's designated outdoor area which is located within the Center's parking lot. LPA observed cones in place to direct cars entering the parking lot limiting them to the first rows closest to the entrance. LPA observed Ms. Syed and the children in the back corner of the lot furthest from the entrance. LPA also immediately observed construction in progress in and around the preschool's fenced outdoor activity area. LPA approached Ms. Syed to inform her of the purpose of the visit and addressed issues observed. Director Syed confirmed that there has indeed been ongoing construction and stated she was unaware that Licensing needed to be notified. LPA accompanied Ms. Syed and the children back to the classroom. Per LPA's request, Office Manager Mr. Saqib guided the LPA on a tour of the preschool outdoor activity area to observe the construction taking place. Mr. Saqib was able to provide detailed information about the construction project which is scheduled from 8/2-8/19. LPA observed the double glass door leading to the outdoor area to be locked as well as the school's clinic and visitor restroom that share the hallway. Per Mr. Saqib all exterior doors are kept locked at all times and all visitors must be buzzed in electronically after being screened through the security camera/intercom system. Per Mr. Saqib, all construction is strictly exterior only and workers must buzz in to use the restroom; at which point they are met by staff who open exterior doors and unlock the restrooms. LPA also observed a set of double doors leading to the primary school and administrative offices to be closed and can only be opened from the inside. Per Mr. Saqib these doors are kept closed while school is in session during the week. Mr. Saqib added that security cameras are placed throughout the entire facility both indoors and outdoors, including the parking lot, all hallways and stairways. During this visit the following was observed and cited in accordance with Title 22, California Code of Regulations. ( See attached LIC 809-D ). The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Director, Shabana Syed via Zoom. A copy of this report along with a copy of the appeal rights and notice of site visit will be sent via email to the Director with a read receipt. An electronic read receipt will confirm the receipt of the report. Director agreed to sign and return original copies to Monterey Park Regional Office.
