What the inspector wrote

Licensing Program Analyst (LPA) T. Tran made an unannounced Case Management Incident visit to the Cesar Chavez Head Start. On 08/19/25, the facility made a self-reported incident to the childcare licensing department regarding a head injury that required medical attention. Upon arrival, LPA met with the Education Specialist. Jackie Ellingson and we toured the facility, LPA observed proper care and supervision and ratio. During today\342\200\231s visit, LPA completed staff and children\342\200\231s files review. LPA obtained the child\342\200\231s record and the doctor's note. LPA conducted interviews with staff, children, and others. Based on the information that was gathered through interviews, on 8/19/25 there were 12 children with 2 staff present when the incident occurred. During the interview with S1 (see LIC 811), during the outdoor play, staff observed C1 went under the play structure and hit the head against the mental edge and sustained an open cut. Staff immediately provided first aid care. School nurse and parents were contacted. C1 was taking to the local health facility for treatment. The child received 3 staples to close the wound. The child returned to school the next day without any physical restrictions from the doctor. At this time there is no violation of Title 22, California Code of Regulations for lack of care and supervision. No further action is required. No deficiency was cited at this time. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, Jackie Ellingson
