What the inspector wrote

Licensing Program Analyst (LPA) Fabiola Vasquez conducted an unannounced case management inspection to follow up on an incident that was reported to the Department on 08/06/21. Upon arrival, LPA met with Director, Julia Weyand, who provided LPA a tour of the facility. There were 46 children and 11 staff present during the inspection. On 08/06/21, an unusual incident report was made to the department regarding an incident that involved a child were the child slipped while pushing a plasma car and sustained an open wound cut on his chin. The facility reported this incident to the Department within the required 24 hours. During this investigation LPA interviewed with staff 5 and 5 children. LPA obtained documentation relating to the incident. Based on the interviews conducted there were conflicts of statements of how the child got injured and through staff's own admission no one saw what happened. The department has determined that there was a lack of supervision which poses an immediate risk to the health and safety of the children in care. California Code of Regulations, Title 22, Division 12, are being cited on the attached LIC 809D. The Licensee was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these forms. A printed copy of this report and appeal rights (LIC 9058) was provided to Director Julia Weyand, at the conclusion of the visit and their signature on this form acknowledges receipt of these forms. The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

This time requiring stitches and was not reported to the Department within the required 24 hours. The Director did not submit a written report within the required 7 days. This poses a potential risk to the health and safety of children in care.
