What the inspector wrote

Licensing Program Analyst (LPA) Shushanik Safaryan conducted an unannounced Case Management inspection for the purpose for the following up on an unusual incident report. Upon arrival at 1 :30 pm , LPA met with Facility Representative , Rebecca Adachi Assistant Director , to whom the purpose of the inspection was explained. Tour of the facility was provided. LPA observed 65 napping children present during the inspection with 7 staff members . On 06/04/24 an unusual incident report was submitted to the Department regarding a child sustaining an injury on the facility grounds. Child was crossing the monkey bars and while crossing fell hitting her face. Child was seen by physician and stich was placed on her bottom lip. LPA conducted review of the play area, took pictures of monkey bars where the injury took place, obtained sign in sheets for staff and children from the day of the injury . LPA observed cameras at the play area. Per Director and Assistant Director cameras were out during the incident . (Declaration Obtained ). LPA advised Facility Representative to add some cushioning to the bench area, to the corners and to the sides . During the inspection , interviews were conducted with five staff members and child . Per Staff #1 , she observed the incident . Staff #1 indicated staff # 3 was closer to the area where incident took place . Staff #2 indicated she did not observe the incident , she heard about it from staff #1 who observed the incident . LPA was unable to interview staff # 3 . Per staff# 5 , she was standing with staff #1 and was observing child when incident took place . Per child staff #1 and staff#5 and one parent were watching her crossing monkey bars . Per staff members child was attended immediately by staff, first aid was provided to the child, and an ouch report was created for the child on the day of the injury (obtained during today's inspection). Based on the information obtained and observation of yard area, LPA determined that there was adequate supervision on the day of injury, and it was not possible to prevent the injury from happening. Per California Code of Regulations Title 22, Division 12, no deficiency cited during today's visit. The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
