What the inspector wrote

Licensing Program Analyst (LPA) Shushanik Safaryan conducted an unannounced Case Management inspection for the purpose for the following up on an unusual incident report. Upon arrival, at 2:30 PM , LPA met with Facility Representative , Rebecca Adachi , to whom the purpose of the inspection was explained. Melanie Burke , Assistant Director came in later .Tour of the facility was provided. LPA observed 72 children present during the inspection with 9 staff members . On 10/24/24 an unusual incident report was submitted to the Department regarding a child sustaining an injury on the facility grounds. Child tripped over on the bench in the tabletop area of the preschool yard and fell forward hitting his forehead on the table . Child was seen by physician and 3 stiches were placed. During the visit , LPA toured the area were incident took place and observed tables and benches and did not observe any hazards . Staff who observed the incident was not present during the visit . There were 56 children present with 9 staff members when incident occurred .LPA interviewed staff present during the incident. Per S1 incident happened when C1 was done with his snack and was ready to go back to play when tripped over the bench and hit his forehead on the table . Per staff members child was attended immediately, first aid was provided to the child, and an ouch report was created for the child on the day of the injury (obtained during today's inspection). Staff stated C1 was back to school next day without any restrictions . Based on the information obtained and observation of yard area, at this time, there is not a preponderance of evidence that shows that the facility was in violation with Title 22 Regulations when these incidents occurred. Therefore, there are no deficiencies being cited. The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
