What the inspector wrote

Licensing Program Analyst (LPA) Stephanie Li conducted an unannounced Case Management inspection due to an incident that occurred on 7/11/2025 through 7/25/25 and was reported to the Department. A risk assessment was conducted. LPA met with Site Supervisor Melanie Burke, to whom the reason for the visit was explained. Director guided LPA on a tour of the facility. There were 0 children present and 9 staff. The facility was conducting in service and no children were present. On 7/11/2025, an unusual incident report was made to the Department regarding an HFM outbreak in the infant cherry blossom room. Additional cases were reported as they were confirmed. Facility reported to their local Pasadena health department. Infants with positive HFM had siblings in their preschool classrooms and facility had confirmed cases on their preschool side. Confirmed HFM cases occurred in clusters in the different classes. Pasadena Health provide responses to facility and provided guidelines for cleaning. Facility implemented cleaning, screening, and prevention guidelines per Public Health. During todays visit, LPA interviewed Director regarding the outbreak, inspected classroom cleaning guidelines, verified reporting to public health and their responses and cleaning instructions, and verified cleaning solution in use. Per site supervisor, one child was sent home with suspected case, returned cleared by doctor but had HFM, which prolonged potential exposure to others. LPA observed the cleaning protocols, notice to parents and policy for returning to school after HFM. Printed copies were provided to LPA. *continued on next page........ Based on information obtained during this investigation, no follow up is necessary regarding the incident reported. No deficiencies are being cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Site Supervisor Melanie Burke.
