On October 29, 2025, Licensing Program Analysts (LPAs) Monique Ayala and Mariah Aguirre conducted an unannounced Case Management- Incident inspection at the above facility. LPAs met with assistant director, Melanie Burke who guided LPAs on a tour of the facility. LPA observed 77 children with 10 staff members The purpose of the inspection is to follow up on an incident report that occurred on 10/24/2025 and was reported to the department on 10/24/2025. During the inspection, LPAs interviewed Staff #1 (S1). LPAs were unable to interview Staff #2 (S2) as S2 was not present at the facility. LPAs attempted to interview Child #1 (C1) but, C1 did not want to interviewed. LPAs interviewed Parent #1 (P1). LPAs obtained a copy of internal incident report and other relevant documents. There are no deficiencies being cited at this time as further investigation is needed. An exit interview was conducted and a copy of this report was provided to Melanie Burke along with Notice of Site Visit. Notice of Site Visit must be posted for 30 days.
