What the inspector wrote

On April 28, 2026, Licensing Program Analyst (LPA) Monique Ayala conducted an announced Case Management - Incident inspection at the above facility. LPA met with Assistant Director, Rebecca Adachi who guided LPA on a tour of the facility. LPA observed 95 children with 13 staff members. The purpose of the inspection is to follow up on two incident reports that were reported to the department; both incidents were reported timely. During the inspection, LPA interview Staff #1 (S1) to Staff #2 (S2) for the first incident and interviewed Staff #1 (S1) for the second incident. LPA did not interview children for either incident as children were napping. The first incident occurred on 03/18/2026, where C1 was playing on the playground engaged in a dance activity. Per S1, C1 was dancing with a group of approximately 10 children when S1 stated they turned around and heard a loud thump on the ground. Per S1, when they turned they saw C1 on the ground holding their head and stating the front of their head hurt. Per S1, another staff member provided an ice pack to S1. S1 stated C1's parents (Parent #1 (P1)) were called and informed about the incident. Per S1, there was nothing on the ground that may have caused C1 to trip/fall. Per S2, P1 arrived at the facility at 12:30pm and took C1 to the doctor where it was confirmed C1 had a minor concussion. Per S1-S2, C1 returned to the facility the following day with no restrictions. LPA advised S1-S2 that when doing a group dance activity to remind the children about personal space and being aware of their surroundings. The second incident occurred on 04/13/2026, where C1 was running in the classroom, slipped on spilled water and sustained a cut on the bottom of their chin. Per S1, they were assisting children with putting their sweaters on as the classroom was going outside to play. Per S1, they told C1 to stop running and use their walking feet in the classroom. S1 stated as C1 ran around a table they observed C1 slip and land on their stomach. S1 stated C1 came over to them, S1 asked if C1 was okay and proceeded to placing their sweater on as S1 saw C1 started to bleed from their chin. S1 stated they called for assistance and started applying pressure to C1 chin. Per S1, C1 received stitches on their chin and returned to the facility the following day. There are no deficiencies being cited at this time. An exit interview was conducted and a copy of this report was provided to assistant director, Rebecca Adachi. A Notice of Site Visit was provided and must be posted for 30 days.

    LIMITATIONS ON CAPACITY

    Limitations on Capacity: A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement was not met as evidence by: The facility had 95 children present when the overall capacity is 93 children. This poses an immediate health and safety risk for children in care.
