What the inspector wrote

On April 28, 2026, Licensing Program Analyst (LPA) Monique Ayala conducted an announced Case Management inspection at the above facility. LPA met with Assistant Director, Rebecca Adachi who guided LPA on a tour of the facility. LPA observed 95 children with 13 staff members. LPA informed the assistant director that the facility was operating out of capacity as the facility is only licensed for 93 children and there are 95 children present. LPA informed the assistant director that two children needed to be picked up to have the facility in compliance. LPA informed the assistant director that the facility is being cited a Type A deficiency in accordance with Title 22 Regulations, see LIC809D. LPA Ayala informed the assistant director, Rebecca Adachi to provide a copy of this licensing report dated 04/28/2026 that documents any type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report, A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child’s file for verification. A Notice of Site Visit was provided and must be posted for 30 days. An exit interview was conducted and a copy of this report was provided to the assistant director, Rebecca Adachi.

    Limitations on Capacity: A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement was not met as evidence by: The facility had 95 children present when the overall capacity is 93 children. This poses an immediate health and safety risk for children in care.
