On June 26, 2024 at 2:20 pm, Licensing Program Analyst (LPA) Angelica Wallin conducted a Case Management Incident inspection. This inspection is regarding an incident that took place on June 12, 2024 in which a child, while running on the playground, tripped and fell into the black metal gate surrounding the outdoor playground and sustained a small open wound on back of the head. LPA met with director, Yvette Camacho, who provided information and assistance during inspection. A census of 16 children and two staff were taken. During the course of the inspection, LPA conducted interviews with two children, one staff and one parent. During inspection, LPA conducted children and staff record review. During inspection, LPA evaluated outdoor play area where incident occurred. LPA observed age-appropriate playground area and toys for children in care. Per director, child was running from one side of playground to the other and in trying to slow down, tripped over feet and fell into the rods of the black metal gate. Per staff interview, director maintained direct supervision during the incident and provided immediate assistance. Per children, staff and parent interviews, it was confirmed that child sustained injury by running on playground and falling into black metal gate surrounding playground area. LPA observed outdoor play area is enclosed and no hazards present to children in care. Based on the available information: observations, interviews with children, parent and staff, and record review, no Title 22 deficiencies cited. LPA discussed with director, preventative measures to be implemented during outdoor play. Per director, there is a no running policy to prevent trips and falls. Per director, children are reminded daily during outdoor play regarding rules of outdoor play. During inspection, LPA provided consultation regarding updating outdoor safety policy regarding closed toes shoes with traction and updating outdoor artificial turf mats to prevent any further tripping hazards. The content of this report was read and discussed in detail at the time of inspection with director, Yvette Camacho. An exit interview was conducted, a notice of site visit to be posted for 30 days provided.
