What the inspector wrote

Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced Annual Random Inspection on this date 5/2/25. Upon arrival at 12:05pm LPA met with Licensee Triwa Schrock, informed of the purpose of inspection and provided form LIC 126, Inspection Entrance Checklist. The Licensee provided tour of facility. LPA inspected rooms/areas on the facility sketch in which child-care services are provided and to which children have access. Per licensee the hours of care provided are Monday \342\200\223 Sunday 6:00am - 7:00pm. The licensee offers overnight care Monday -Sunday 6:pm - 6:00am, LPA discussed with licensee that care provided during the day and overnight combined shall not exceed 24 hours from the time the child entered care. There were 3 children present, 1 infant and 2 - two year olds. Also present was Kayla Johshan, Assistant. All adults present have obtained the required background clearance and are associated to the facility. Individuals residing in the home were discussed and noted. This is a two-story home which consists of three (3) bedrooms, two (2) bathrooms, kitchen, dining room, living room, front yard (not fenced), backyard (fenced) and attached garage (locked). Childcare areas are in the living room and backyard. The living room is divided in half by a baby gate LPA observed the connections of the baby gate to the wall has created a small holes exposing the drywall this poses a potential risk to the safety of children in care. Per Licensee, areas off limits to children are the entire second floor of the home which includes three bedrooms and one bathroom, attached garage, kitchen side yard. LPA observed a safety gate at the bottom of the stairs that separates the off limits second floor from the first floor. Page 1 \342\200\223 Report Continues LPA also observed a safety gate that separates the dining room from the off limits kitchen. The children use the bathroom located in the dining room area. The restroom that children use was observed to be safe and sanitary. LPA observed the facility license, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form posted in wall in living room/daycare room. LPA reviewed Children\342\200\231s Roster form LIC 9040 and Emergency Disaster Plan (LIC 610A) The home is observed to be orderly. LPA observed the Smoke and carbon monoxide detectors were tested and are operable. Fire extinguisher indicated fully charged purchase receipt date 9/20/24.The home maintains telephone service via cell phone line. There are toys and other age-appropriate material available. LPA observed that cleaning compounds are in locked cabinet in off limit kitchen. Licensee was reminded that the areas used for daycare must be cleaned regularly. Licensee states that there are no poisons stored in the home and understands that all poisons must be locked, not only inaccessible to children. Per Licensee there are no firearms or weapons stored in the home. Per Licensee, area for used for when children are ill is in living room supervised and separated from the other children. Outdoor play area is in the yard and observed to be fenced. LPA observed that the outdoor yard has toys and other age appropriate materials. The ground is dirt and covered with artificial turf. The facility does not have a pool or similar bodies of water. Per licensee there are no pets in the home. No pets were observed. Three (3) Children\342\200\231s records were reviewed for required forms and documentation. Licensee and Staff records were reviewed for required forms and documentation. LPA observed Staff was missing LIC 9052 Employee Rights, completed the LIC 9052 during this visit. The licensee stated transportation is provided to children in care. Staff and licensee records are complete during file review. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. --------Page 3 -Report continues Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LPA reminded licensee that gliders, soothers, rockers and swings are not safe for infant sleep due to risk of suffocation - PIN 21-19-CCP was provided. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Based on the LPA's observations and records review 2 B deficiencies will be cited today in accordance with California Title 22 Regulations. One B deficiency was cited during this inspection in accordance with title 22 regulations. A notice of site visit was given and must remain posted for 30 days.
