What the inspector wrote

On Monday, December 8, 2025, Licensing Program Analyst (LPA) Staicy Perry conducted an unannounced case management inspection to follow up with an incident that was reported to the department. LPA met with Director Sakuntala Abeyasekera who guided LPA Perry on a tour of the facility. Census was taken. On December 1, 2025, it was self reported to the department regarding C#1. The facility reported that on November 24, 2025, the parent informed director that Child #1 had been complaining of side pain, and the pain persisted throughout the week. On November 30, 2025, the parent took Child #1 to Urgent Care, where the child was medically evaluated and diagnosed with a vaccine preventable disease. Child #1 was present at the facility on November 21, 2025. Child #1 was absent from November 24–25, 2025 due to feeling ill. The facility was closed from November 26–28, 2025 for the Thanksgiving holiday. The facility stated that Child #1 did not have any symptoms or complain of pain while in care. C#1’s file was observed and all vaccinations were observed. Per facility director it was reported to the Los Angeles County Department of Public Health, Vaccine Preventable Disease Control Program and spoke to Khemara Has via telephone on 12/1/2025. The facility provided LPA Perry with confirmation of the email and conversation with DPH and guidance received from Public Health regarding vaccine preventable diseases. The facility also implemented enhanced cleaning measures. The facility notified the Department in a timely manner once they became aware of the situation. Child #1 has returned cleared to the facility on 12/8/2025 with a doctor’s note indicating no restrictions. The facility documented the incident and reported it to Community Care Licensing as required. Based on the information obtained, the facility is in compliance with Title 22 regulations. No deficiencies are being cited today. A copy of this report was provided to the Director Evelyn along with the Appeal Rights. A NOS was provided to the Director Sakuntala Abeyasekera and was reminded that it must be posted for 30 days
