What the inspector wrote

On 04/14/2026 at 8:30AM Licensing Program Analyst (LPA) Mariah Aguirre conducted an unannounced Annual/Random inspection at the above facility. Upon arrival, LPA was greeted by licensee, Xiuling Hu who did not speak English. Per Licensee they only speak Mandarin. LPA MAguirre attempted to contact the CTS Language Link and the call went to voicemail twice. LPA MAguirre then contacted support staff Youle Qi who assisted LPA with translation throughout the inspection. Licensee Xiuling Hu guided LPA on a tour of the facility. LPA observed 11 children in care, four of which are infants. Per licensee there are 13 children enrolled. Hours of operation are Monday through Friday, 7:30AM to 6:00PM. Four adults live in the home. This is a single-story house with 4 bedrooms and 3 bathrooms, living room, dinning room, family room, kitchen, rear yard, garage. Area's accessible to children in care are the living room, dinning room, family room, hallway bathroom and rear yard. All other areas are made in accessible by locked doors. LPA observed fire place to be in compliance with Title 22 Regulations. LPAs observed napping equipment for children in care in family room and living room (playpens and cots). Per Licensee, parents provide bedding and it is taken home every week to be washed. Per licensee there are no bodies of water on the premises, LPA did not observe any. Per licensee there are no pets, LPA did not observe any at the time of inspection. At the time of inspection, LPA MAguirre requested to review the 15 minute sleep log for all infants in care. LPA MAguirre and Support Staff Youle Qi asked the licensee why it was taking a long time to provide the logs, per Licensee, she did not complete the sleep log for certain days and was trying to fill out the days missed. As a result, one type B citation was issued. (See attached LIC809D). Page 1 of 5 The home is clean, orderly, comfortable, and well ventilated. Licensee provides AM snack, lunch and dinner to children in care. Per licensee, she is a part of a food program through Options for Learning. LPA observed a functioning Smoke detector and Carbon Monoxide and working telephone. LPAs observed fire extinguisher to be fully charged and serviced. There are several age-appropriate toys and a first aid kit on the premises. Per the licensee, there are no firearms on the premises. Licensees First Aid and CPR expires on 03/2027. All areas identified on the facility sketch that are accessible to children were inspected for safety, comfort, and cleanliness. Home is well ventilated. Licensee provides lunch, dinner, and AM snack to children in care. Per licensee, they are a part of a food program through Options for Learning. LPA observed all cleaning products to be kept in the bathroom on the top shelf of a storage space, inaccessible to children in care. Per licensee, there are no poisons in the home. LPA observed the knives kept in a high kitchen cabinet that is inaccessible to children in care. LPA reviewed children’s files and files were found to be incomplete, LPA did not observe an LIC9227 to be in any infant files. LPA could not review Licensee or additional staff files as Licensee states no staff have a file. As a result, three type B citations were issued (See attached LIC809D) Children, Licensee, and Staff names were documented. LPA observed a current facility roster. The licensee has completed the online mandated reporter training at www.mandatedreporterca.com , expires on 01/07/2028. All childcare employees must complete mandated reporter training within 30 days of hire and renew training every two years. Per Licensee, 2 staff do not have a Mandated Reporter Training Certification. As a result, 1 type B citation was issued. (See attached LIC809D). Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Page 2 of 5 Licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC624B. The licensee was informed of the responsibility to report suspected Child Abuse by calling the Child Abuse Hot-line at 1-800-540-4000. Also call the CCLD office and follow up with a written Unusual Incident/Injury Report (LIC 624B). Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Per the Licensee, fire and disaster drills are conducted every six months and the last drill documented and conducted was April 15 th 2026. LPA MAguirre observed a current drill log at the time of inspection. Licensee has the required documents posted in the FCCH; Facility License (LIC 203), Emergency Disaster Plan (LIC610a), Notification of Parents' Rights Poster (PUB 394). The following was discussed with the licensee: Capacity requirements, Roster requirements, Posting requirements, Documentation requirements for disaster drills (fire and earthquake). Mandatory Forms for the children’s files and provider’s files, and Safe Sleep Regulation. The role and responsibilities of being a mandated reporter were reviewed. Licensee was reminded that supervision is always required to children in care. Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must always have the facility’s phone number; if the phone number is changed, licensing must be notified. Page 3 of 5 Regulation prohibits the smoking of tobacco in a private residence that is licensed as a family childcare home and in those areas of the family day care home where children are present (24/7 ban). State law prohibits baby walkers, bouncy seats, exersaucers and any other items that fall into that category. --Licensee was advised to visit the CCL website ( www.ccld.ca.gov ) to obtain updates of courses and updates/changes to the regulations. -- Our Quarterly updates come out every 3 months they are also now in Spanish please log in to the CCLD website or you can email our advocates to have the quarterly updates send directly to your email. Child Care Advocates information: www.childcareadvocatesprogram@cdss.ca.gov The on Duty Worker is available for questions Monday through Friday at (323) 981-3350 from 8:00 AM - 5:00 PM. LPAs discussed the safe sleep regulations with the Licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA reviewed the 15 minute sleep log kept for one child in care, sleep log was found to be complete. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee, Xiuling Hu confirmed that there are no Registered Sex Offenders living in the home and LPA completed the RSO profile in FAS.
