What the inspector wrote

Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced case management inspection on 01/09/24 at 1:30 PM due to an anonymous email that was received on 12/08/23. LPA met with Elisha Gonzales, Director who guided analyst on a tour of the facility. LPA obtained December 2023 Illness Notifications and staff workplace illness Policy. At this time, there is not a preponderance of evidence that shows that the facility was in violation with Title 22 Regulations based on 12/08/23 anonymous email was was received. Therefore, there are no deficiencies being cited. The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the director, Elisha Gonzalez at 3:00 PM.

Based on staff interviews, on 01/02/24 approximately from 9:46 a.m. to 9:49 a.m., child#1 was alone and unsupervised on outdoor yard. This poses an immediate health, safety and personal right risk to the children in care.
