What the inspector wrote

Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced case management- incident inspection on 06/16/25 at 11:20 AM, due to an incident that occurred on Wednesday, 05/14/25. LPA met with Elisha Gozalez, Director who guided analyst on a tour of the facility. During this inspection 41 infants were present with 21 staff. Alleged Incident took place on Wednesday, 05/14/25 at 5:24 PM. Per Stephanny Lemus, Health and Safety Director, she contacted Regional Office on Thursday, 05/15/25 at 5:19 PM and left a voicemail. Incident was reported via telephone on Friday, 05/16/25 at 8:21 AM. The written incident report was received during 05/23/25 inspection. The facility reported the incident within the required 24-hour time frame. During this inspection LPA Eivazian reminded director that Regional Office is open Monday to Friday from 8:00 AM to 5:00 PM. LPA observed the area where alleged incident took place. Pictures were taken. LPA conducted interviews with three staff. Based on three staff interviews, on 05/14/25 at about 5:24 pm, end of the day, as a table activity was ending, child#1 ran from one end of the table to the other and tripped on a chair causing child#1 to fall forward and bump left eyelid on a nearby shelf and sustained a cut on left eyelid. Per staff #2, she observed how the incident occurred, she was about to stop child#1 from running but the incident occurred so quick. Per staff#2, first aid was applied, and child#1 received 5 stiches. On 05/23/25, LPA Eivazian assessed where the incident occurred and did not observed any pointed or hazards material in the classroom, on the shelf where the incident occurred. At this time, there is not a preponderance of evidence that shows that the facility was in violation with Title 22 Regulations when these incident occurred. Therefore, there are no deficiencies being cited during this inspection. The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Elisha Gonzalez, Director.
