What the inspector wrote

On 05/20/26, at 12:00 PM, Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced case management- incident inspection to the above facility due to an incident that occurred on Monday, 05/04/26. LPA met with Elisha Gozalez, Director who guided analyst on a tour of the facility. During this inspection 40 infants were present with 23 staff. A preschool program (198020405) on the same premises with total capacity of 182. Alleged Incident took place on Monday, 05/04/26. Incident was reported via telephone on the next working day, 05/05/26. The written incident report was received via email on 05/08/26. The facility reported the incident within the required 24-hour time frame. On 05/04/26, an incident took place where child #1 was walking in the yard and lost balance and fell forward and hit bottom lip on a wood planter and sustained a cut on bottom lip and received 3 stitches. On 05/20/26, LPA observed the area where alleged incident took place. Pictures were taken. LPA conducted interviews with three staff. LPA obtained a copy of doctor note for child#1 dated 05/12/26. Sign in/out sheet for 05/04/26 for the classroom where the incident occurred. Per staff # 1, who witnessed the incident, on 05/04/26, at 4:32 PM, child#1 was on the yard, walking near the perimeter of the fence when lost balance, fell forward and bumped bottom lip on a wood planter and sustained a cut on bottom lip. Per staff#1, she checked on child#1 right away and once she observed child#1 was bleeding asked for help. Per staff#1, staff#4 cleaned the area, and applied pressure. Office was called and staff#2 observed the injury and contacted parent#1. PAGE 1 of 2 Per staff#2, once she observed the bleeding was stopped on child#1’s lip and was calm, contacted parent#1 and asked to come look at the injury as it seemed it may need to be checked by a physician. Parent#1 arrived at 5:23 PM, and waited till parent#2 arrived, who had a car seat and left the facility at 5:56 PM. Per staff#3, she contacted parent#2 at 8:30 PM at night on 05/04/26, who stated they took child#1 to Urgent Care and child#1 received three stiches on bottom lip. Per staff#3, child#1 was out for two days and came back on 05/07/26. LPA observed where the alleged incident occurred and did not observe any hazards. Per director, school added rubber cushioned on the edge of the wood planter the next day of incident to prevent injuries in the future. Ratio and staffing were in accordance of Title 22 Code of Regulations at the time of the incident. At the time of the incident there were 10 children with 6 staff in the classroom where the incident occurred. At this time, there is not preponderance of evidence that shows that the facility was in violation with Title 22 Regulations when these incident occurred. Therefore, there are no deficiencies being cited on this date. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the director, Elisha Gonzalez. REPORT End 4 OF 4
