What the inspector wrote

Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced case management inspection on 07/30/25 at 12:40 PM, due to an incident that occurred on 06/05/25. Upon LPA’s arrival to the facility LPA met with Elisha Gonzalez, director who guided LPA on a tour of the facility. The licensee also operates an infant program (198020404) on the same premises. During this inspection 104 children were present in the facility with 30 staff. Alleged Incident took place on 06/05/25. Incident was reported via phone next day on 06/06/25. Original LIC 624 Unusual Incident/Injury Report form was received by the Department within 7 days. The written incident report was received by email on 06/11/25. The facility reported the incident within the required 24 hour time frame. LPA observed the area where alleged incident took place. (Pictures were taken). LPA obtained a copy of child#1 transition report for 06/05/25 and incident report. LPA conducted an interview with staff#1 who observed the incident. Per Elisha Gonzalez, director at the time of the incident, 20 children with 7 staff were present in the classroom where incident occurred. Based on staff#1 interview, on 06/05/25 approximately at 11:03 AM, child#1 was on the balance beam on the twos yard when mistepped, fell forward and bumped forehead on the beam in front and sustained a cut on forehead. Pressure applied to area and bleeding stopped and put band aide. Child#1 parent was notified at 11:12 AM and child#1 was picked up at 11:36 AM. Child#1 was taken to Emergency room and had injury glued. Per Elisha Gonzalez, director, child#1 returned to school next day. REPORT CONTINUES ON NEXT PAGE 1 OF 2 LPA accessed where alleged incident occurred and did not observe any hazards. Pictures were taken. Ratio and staffing were in accordance with Title 22 Code of Regulations at the time of the incident. LPA issued the Confidential Names List (LIC 811) to the licensee during this inspection. The Confidential Names List documents the staff and children involved with the incident documented in this report. At this time, there is not a preponderance of evidence that shows that the facility was in violation with Title 22 Regulations when this incident occurred. Therefore, there are no deficiencies being cited on this date. The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Elisha Gonzales, director. REPORT END 2 OF 2
