What the inspector wrote

Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced case management inspection on 11/14/25 at 8:30 AM, due to an incident that occurred on 11/10/25. Upon LPA’s arrival to the facility LPA met with Elisha Gonzalez, director who guided LPA on a tour of the facility. The licensee also operates an infant program (198020404) on the same premises. During this inspection 52 children were present in the facility with 21 staff. Alleged Incident took place on Monday 11/10/25. Incident was reported via phone next working day on 11/12/25. Original LIC 624 Unusual Incident/Injury Report form has not sent to the Department yet. Director was reminded to submit written report within seven days of incident. The facility reported t he incident within the required 24 hour time frame. Alleged incident is going to the be investigated by the department IB Investigator, Jorge Rojas. LPA obtained a copy of facility roster. Due to insufficient information available at this time, the above incident needs further investigation. At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today from the areas that were inspected today. The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Elisha Gonzalez, Executive Director.
