What the inspector wrote

On 05/26/2026 Licensing Program Analyst (LPA) Mary Silva conducted an unannounced case management inspection. Upon arrival LPA met with supervisor Vanessa Burgueno, who guided LPA on the tour of the facility. The purpose of the inspection was to obtain additional information from the facility regarding an incident reported to the regional office. Census was taken. LPA observed 54 children with 13 staff. The department received an incident report via email from this facility on 05/19/2026 for an incident that occurred at the facility on 05/08/2026 regarding a staff member alleged to have witnessed another staff speak to a child in an aggressive manner. During the inspection LPA MSilva conducted interviews with 3 staff regarding the allegation. LPA obtained documentation in the form of facility roster, student information form and other documentation pertaining to the incident. Based on the information that was verified during the inspection and the information on the incident report, a subsequent follow-up visit will be conducted at a later date as further information is needed. The notice of site inspection must remain posted for a period of 30 days during hours of operation. Failure to maintain posting will result in a civil penalty of $100.00 dollars. Appeal rights were given. Exit interview was conducted and report was reviewed with Supervisor Vanessa Burgueno.
