What the inspector wrote

On 07/03/2025 Licensing Program Analyst (LPA) Mary Silva conducted an unannounced case management inspection. Upon arrival LPA met with director Mildred Bautista, who guided LPA on the tour of the facility. The purpose of the inspection was to obtain additional information from the facility regarding an incident reported to the regional office. Census was taken. LPA observed 30 children and 6 staff. The department received an incident report via email from this facility on 06/30/2025 for an incident that occurred at the facility on 06/27/2025 regarding a child alleged staff violated their personal rights while in care. Per Director,on 06/27/2025 a police officer visited the facility for the purpose of investigating a complaint from a parent. During the inspection LPA conducted (3) interviews with staff and director regarding the allegation. LPA obtained documentation in the form of facility roster, observation forms and other documentation pertaining to the incident. Based on the information that was verified during the inspection and the information on the incident report, a subsequent follow up visit will be conducted at a later date as further information is needed. No deficiencies are being cited during today\342\200\231s inspection. The notice of site inspection must remain posted for a period of 30 days during hours of operation. Failure to maintain posting will result in a civil penalty of $100.00 dollars. Appeal rights were given. Exit interview was conducted and report was reviewed with director Mildred Bautista.
