What the inspector wrote

On 08/21/2025 Licensing Program Analyst (LPA) Mary Silva conducted an unannounced case management inspection to follow up on an incident report submitted to the department on 06/30/2025. This is a subsequent inspection regarding the incident that occurred on 06/27/2025. Upon arrival LPA met with director Mildred Bautista, who guided LPA on the tour of the facility. Census was taken. LPA observed 33 children and 10 staff. The purpose of this inspection is to follow up on an incident that occurred at the facility on 06/27/2025. An investigation was conducted in regards to a possible Personal Rights violation. During the investigation LPA conducted interviews with staff, director and daycare children regarding the incident, video footage was reviewed. Documentation's in the form of facility roster, observation forms, police report, and other documentation pertaining to the incident were obtained. Based on the information obtained during the investigation, it was determined that there was no violation of California Title 22 regulations at this time. The notice of site inspection must remain posted for a period of 30 days during hours of operation. Failure to maintain posting will result in a civil penalty of $100.00 dollars. Appeal rights were given. Exit interview was conducted and report was reviewed with director Mildred Bautista.
