What the inspector wrote

On 01/08/2026, Licensing Program Analyst (LPA) Mariah Aguirre conducted an unannounced Annual/Random inspection at the above facility. LPA met with Director, Minerva Carrion who guided LPA on a tour of the facility. This is a Preschool program that operates Monday to Friday from 7:00AM to 6:00PM. During the inspection, LPA observed 29 children in care with 6 staff members. The following classrooms were inspected: Classroom 1 (room 16): 19 children with 3 staff, Classroom 2 (room 17): 10 children with 3 staff. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision, including visual supervision. Licensed facility is within the conditions, limitations, and capacity specified on the license. Per Director, the isolation area for children is located in the main office. The classroom furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are free of toxins. LPA observed napping equipment in both classrooms. Napping equipment provided are cots. Parents can provide napping equipment for children in care but also have the option to have the facility provide napping equipment for children in care. Per lead teacher, blankets and sheets are sent home and washed every Friday. Page 1 of 4 The facility does not provide meals to children in care. Parents have the option to purchase meals through Choice Lunch, a third party hot meal program or provide lunch from home for the children. The facility does provide AM and PM snacks to children in care and filtered water from faucets in the facility. Per Director and the Child Care Lead Sampling and Reporting Tool, lead testing results were conducted last in 2024, and came back normal. Cleaning solutions were not observed to be in the classrooms accessible to children in care. LPA observed that there are functioning smoke and carbon detectors in the facility. LPA observed a fully charged and serviced fire extinguisher last serviced on 05/13/2025. Emergency disaster drill was last conducted on 01/07/2026. Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with wood chip that absorbs a fall. There is adequate shade in the play yard. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. LPA reviewed staff records, staff files were complete. Children\342\200\231s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. All children's files were complete. Verification of CPR/First Aid and health preventative practices documentation was reviewed. SB792 (Immunization Requirements for Staff and Employees) was discussed with the Director. The Director and staff do currently have immunization documentation. AB1207 Mandated Child Abuse Reporting \342\200\223 On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA discussed the safe sleep regulations with Director Minerva Carrion and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed Director of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. LPA referred licensee [or facility representative] to the Department website for lead: Lead Toxicity Prevention and Water Testing Information ( https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information ) Page 3 of 4 LPA advised the Director to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov . Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. The Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Director Minerva Carrion, confirmed that there are no Registered Sex Offenders working in the facility and LPA completed the RSO profile in FAS.
