What the inspector wrote

Page 2 Reporting Party (RP) alleged: Infant sleeping in crib with a stuff animal, infant was bottle fed laying flat inside the crib, infant was laying in the crib wrapped with a blanket. Licensee stated mom told her to keep the Minnie doll in the pack n play when the child is sleeping. Based on Photos, video, Interviews and LPA own observations of blankets, sheets not fitted and stuffed toys in the infants pack and play, while infant sleeping, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12, Chapter 1, the following deficiencies are being cited (see attached 9099D). Upon receipt, the Licensee shall post the \342\200\234D\342\200\235 page of the Licensing report. This page shall be posted for 30 consecutive days. Failure to maintain posting as required will result in a $100 civil penalty. A copy of this report shall be provided to the parents/guardians of the children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parents/guardians of any children newly enrolled at the facility for the next 12 months. The LIC 9224 Acknowledgement of Receipt of Licensing Reports must be maintained in each child's file immediately upon receipt from the parent. LPA provided Licensee with a blank copy of the LIC 9224 Acknowledgement of Receipt of Licensing Report. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Licensee Iona Ferdinandus.

    Infant Safe Sleep: (3) Mattresses shall be firm and covered with a fitted sheet that is appropriate to the mattress size, fits tightly on the mattress, and overlaps the underside of the mattress so it cannot be dislodged. (b) Cribs or play yards shall be free from all loose articles and objects. The requirement is not met as evidenced by: Photos, video, Interviews and LPA own observations of blankets, sheets not fitted and stuffed toys in the infants pack and play, while infant sleeping. This poses an immediate health, safety or personal rights risk to persons in care.
