What the inspector wrote

Licensing Program Analysts (LPA) Cynthia Reyes conducted a case management inspection due to an incident that occurred on 10/02/2025. This inspection is to ensure the health and safety standards as required by the regulations governing child care centers are met. LPA met with Director Robin Davis who took LPA on a tour of the day care inside and out. This is an incident the facility is stating that a child fell and bumped the back of his head on a piece of play equipment. Per interviews conducted and documents received, child #1 climbed on the play structure on the outside play ground and fell off and hit the ground or step with the back of his head. The child got a cut on the back of his head and ended up going to emergency and received 2 staples. Per interviews there were 3 staff with 23 children on the play ground at that time and the staff were attending to other children at the time the child had climbed the structure and fell off. Some children were transitioning to dance class also at that time. Per interviews no staff saw the child climb to the top of the structure, they only heard when the child was crying after the fall. The facility followed all proper procedures per the parent hand book/admission agreement, incident reports were called and sent in properly and timely, all medical needs were met if needed and parents notified. Based on information obtained for this Incident on this date, no follow up is necessary. Consultation regarding play structure was given to have it blocked off when no staff is near the structure to observe children playing on it. No citation was given on this date. Doctor note and ouch report will be emailed. Exit interview and consultation was conducted with Director Robin Davis and a notice of site visit and appeal rights explained & provided.
