Follow-up on corrections·May 31, 2024

On May 31, 2024, Licensing Program Analysts (LPAs) Carolyn Tuba and Priscilla Ochoa conducted a case management inspection due to a prior visit conducted on 5/3/2024. A COVID risk assessment was conducted. LPAs met with Director, Maria Flores and observed 8 children with 2 staff members outside on the playground. During a prior visit done on 5/3/2024 LPA, Carolyn Tuba was interviewing the Director and asked about the LIC 9224 Acknowledgment of Receipt of Licensing Reports, if parents from the preschool component had signed and been given the two (2) Type-As that had been issued on 3/18/2024 and on 3/25/2024, along with the Licensing Reports. Director stated that only a few had signed them but didn’t have any further explanation. LPA explained that they require signature from parents for each child enrolled and for any new enrollments for an entire year from the date that the citation was issued. The following deficiency listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiency that are being cited need to be cleared to protect the children’s health & safety. One (1) Type A - A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Licensee/Director was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit. LPA informed Director that an office meeting will be scheduled with the Department, at a future date and time and will be held at the Regional Office. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, Maria Flores.
