What the inspector wrote

On 9/6/2024, at 1:00 pm, Licensing Program Analysts (LPAs) Carolyn Tuba and Priscilla Ochoa conducted an unannounced Annual inspection. LPA entered facility through an iron gate which requires a code before entering the facility. There are cameras on the outside premises of the facility grounds. Doors of the classrooms are kept locked for safety purposes. LPAs met with Director, Maria Flores. A Covid Risk assessment was conducted. LPAs were guided on a tour of the facility indoors and outdoors. This is an infant program with a toddler component licensed for 17 children including a preschool and school-age programs. The program operates Monday through Friday from 7:00 AM to 6:00 PM. The Director stated there are 4 children currently enrolled in the infant program and there are no toddlers at this time. The facility is in a residential area. LPAs observed 3 infants with 1 staff and 1 volunteer. The following infant classroom was inspected. All staff members are fingerprint cleared. Licensed facility is within the conditions, limitations, and capacity specified on the license. The facility has additional staff members to support and assist with preparing meals and distribute to classrooms, as well as any other staff and children\342\200\231s needs. The infant room has a separate space with 3 infant cribs and a pack-n-play with firm mattresses. LPAs advised that child\342\200\231s name should be on the crib and that if the child is 12 months and older then they may be place on a cot/mat for napping. Napping cots are available. Fitted sheets are supplied by the facility and laundered weekly and as needed. LPA observed the 15-minute sleep documentation, as well as the feeding and changing logs. LPAs advised that the 15 minute sleep log must be conducted for children 24 months and younger. The classroom furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. Indoor classroom was inspected to ensure that the floors have a surface that is safe and clean. Telephone service, heating, lighting, and ventilation were evaluated. There is storage for children\342\200\231s belongings. Electrical outlets were observed to be covered with child proof covers. Page 1 of 4 Availability of drinking water was reviewed and per Director parents provide water from home and refills are available by the use of bottled water. There is a changing table with arm\342\200\231s length sink for hand washing. Diapers and wipes are supplied by parents. Toys and surfaces are observed to be safe and clean. Staff restroom is available near the kitchen. There is a restroom with 1 sink and 1 toilet in the infant room but is not used. The toddler classroom is not in use and is being utilized for storage of extra toys. LPAs reminded Director prior to utilizing the classroom it be inspected for safety. LPAs observed a changing table with arm\342\200\231s length sink for hand washing. Carbon monoxide and smoke detectors were present in the classroom and are connected to each other throughout the facility. LPAs observed while Director tested and were found to be operable during the time of the visit. Fire extinguishers are serviced annually and was last serviced March 12, 2024. Open shelves in classrooms were inspected and no hazardous items were found, cleaning items are kept inaccessible to children in care. No poisonous cleaning compounds were found in the childcare center. Emergency disaster drill was last conducted on July 16, 2024. There is a First Aid kit available in the classroom; and supplies were inventoried and found to be in good condition. There is a kitchen area where meals are prepared and distributed to the classroom. Food preparation areas were toured for safety, cleanliness, and proper equipment. Breakfast and PM snacks are served by the facility. Lunch Is provided by parents. Extra food is available for children in care. Menu is posted in the parent board of the classroom. LPAs observed parent board with all required documentation located in the entry way. Director informed LPAs that parents provide formula and baby food for the infants in care. There is also a small refrigerator used for bottle storage. LPAs were informed that there are currently no children on medication at this time, however the Director has an IMS plan and LPAs reviewed and provided consultation. Director stated that medication would be kept in the kitchen out of reach of children or in the refrigerator labeled with child\342\200\231s name and expiration date. LPAs advised that medication should be given back to parents/guardians when child leaves the facility or medication expires. Outdoor playground equipment is in safe condition, free of sharp, loose, or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There are currently 4 Page 2 of 4 playgrounds, and each one is designated for each component (school-age, infant, preschool & toddler). The infant playground has play equipment with grassy area and shade is provided by trees. LPAs advised that children should have outside time, however weather permitting with extreme heat advisories, scheduling can be adjusted. LPAs advised that play equipment requires to be kept clean free of bird feces, spider webs and dirt. There is also a toddler playground that has play equipment with sand to cushion falls. The play yard is not being used currently. The playground is surrounded by an iron gate and cement wall at least 6 to 7 feet tall. Sign in/out was available for review. LPAs advised that parents should use full signature when signing in/out their child. LPA reviewed staff records and found that they were complete. Clearance for adults and verification of CPR/First Aid, educational requirements, immunization records and health preventative practices documentation was reviewed. Children\342\200\231s records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. LPAs observed that Director had the require 8-hour Health, Safety, Nutrition and Lead training. LPAs advised of Infant Sleep Plan for children who were enrolled prior to their first birthday and provided a copy. There was also a Needs and Service Plan conducted when entering the program. LPAs advised to conduct every 3 months to infant changes or as needed. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA reminded that lead results need to be posted for parents to view until the next testing. Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. page 3 of 4 To improve the quality and value of the new inspection process, a survey may be sent to the email address provided. There were no citations being issued during this visit. A notice of site visit was given and must remain posted for 30 days.
