What the inspector wrote

Licensing Program Analyst (LPA) Cynthia Reyes, conducted an announced pre-licensing site Inspection to inspect and measure the requested Preschool component with a Toddler Option for capacity determination. LPA met with Heather Warma, Administrator & Lydia Guerrero, Director. Due to COVID- 19 precautionary measures were taken during the entire inspection and all individuals present during this inspection wore appropriate personal protective equipment. The licensee is requesting a capacity of 154 Preschool (2-5) and 15 toddlers (18 to 36 months). Days & Hours for the proposed preschool with toddler option will be Monday- Friday 6:30 AM-6:00 PM. At 8:45 AM, LPA began the tour of the facility with the Administrator and the Director, beginning at the front entrance to the facility located from the parking lot on Sunset Ave. Administrator states parents will enter and exit the gate and will stop at a table in front of the day care office during Covid where parents will sign the child in/out electronically using the pro-care digital system where the system scans their finger. (Alcohol swaps and hand sanitizer will be available), Office staff will take children temperatures and use of hand sanitizer will be conducted. Parents will then walk the child to the class room and not enter the class rooms at this time, unless they request to enter, which at that time all precautionary measures will be taken for that individual entering. Facility staff will walk the child inside the class room and bring the child out at pick up time. Per Director children's temperatures will be taken in the class room every two hours and a log will be kept in each class room. LPA advised the Administrator and Director that a parent board should be placed outside by the sign in/out table visible for all to see with all licensing documents and Covid-19 posting. LPA also advised the Administrator and Director to adhere to the local public health guidelines at all times during Covid. (According to CDC guidelines, children under the age of (2) are not required to wear masks.). LPA requested that photos to be sent of all Covid-19 posting in each area of the school, including the rest rooms. Continued on next page Page 2 The Directors office will be used as an ill/isolation area and has a cot/mat for a child to rest if needed. Ill children will use the staff bathroom located outside the Directors office. The isolation area will be used by both components (Preschool and Toddlers), however the facility will also use the Administrators office as an extra area that can be used if a child from both components are sick at the same time. 9:05 AM, The tour continued outside in the Toddler outdoor play area where measurements were taken and observed by both the Administrator and the Director. The tour then continued to the Preschool outdoor play yard where measurements were also taken. Both Toddlers and Preschool have their own self contained outdoor play yard that they will access though their own class rooms. Water jugs and disposable cups will be used for each component while outdoors. LPA observed in both play yards age appropriate outdoor toys and play equipment and are clean safe and in good repair. Adequate shade was also observed to be available. All areas around play equipment and slides are cushioned with material to absorb a fall. LPA advised the Director that the children need to be within the direct care and supervision, including visual observation and supervision of the teacher(s) at all times. 9:30 AM Licensing staff was guided through the proposed Preschool classrooms that consist of 8 class rooms (Rooms 101/102/201/202/203/204/301 and 302.) LPA was then guided through the Toddler class room that consist of 1 room located in room #103. All rooms need to be labeled with class room numbers outside the class room doors. LPA and Director measured each class rooms (Preschool and Toddlers) together along with the Administrator to determine capacity for each room and for the total capacity requested. In each class rooms (Preschool and Toddlers) LPA observed and inspected, Indoor play equipment, furniture, fixtures and materials for preschool and toddler children to be clean, safe, age appropriate and in good repair. Cleanliness and safety of the facility was also observed. Licensing staff observed children sleeping cots are located in each class room and children sheets and blankets will be located in each of their own cubbies. Parents will bring child own linen. The linens will be taken home weekly on Fridays by child\342\200\231s authorized representative for laundering. Facility has extra linen available if needed. Continued on next page Page 3 Children will use small cups and water jugs that will be available in each class room. There is a fully equipped first aid kit available in each of the classrooms and a main large first aid kit will be located in the Directors office. If any lifesaving medication are needed they will be stored in a locked cabinet in each of the staffs own class room for that specific child. Telephone service, heating, lighting and ventilation were evaluated. LPA observed there are fire extinguishers located right outside each class room and inside each class room are dual smoke detectors with carbon monoxide detectors that meets statutory requirements. No disinfectants, cleaning solutions, poisons and other hazardous items that are dangerous to children were observed to be accessible. This facility will provide AM and PM snacks and parents can bring their child's own lunch or they have an option to purchase a hot lunch from an outside vendor (Cator Tots) used by the facility. Snacks and lunches will be stored in the kitchen only. The kitchen on site is clean, safe and in good repair. A monthly snack menu is available and posted. LPA advised the applicant to label and date all food, dishes and cups that are brought from home for proper storage. No transportation is being conducted by the facility. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. 11AM Items discussed during this Inspection: Licensing does not tolerate the following: Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238 (g) (2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both. Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request. Current Children\342\200\231s Roster LIC 9040: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This children roster must be updated and available to the licensing agency upon request. Page 4 Continued Items discussed during this Inspection: Mandated Reporter Training: requires all licensed providers, applicants, directors and employees to complete training http://www.mandatedreporterca.com/ . Pediatric First Aid and CPR: At least one person in the facility, along with the Director/Administrator must be trained in Pediatric First Aid and CPR and present in the facility. Designated Staff: The name of the childcare center director or fully qualified teacher(s) designated to act in the director's absence must be on file. Qualifications and staff files: Educational background, training, and/or experience and all required documents by the department for each staff present must be available for review. Children\342\200\231s Records: All required licensing documents must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary. Director advised to visit www.shotsforschool.org for immunization information. Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, & measles. Criminal background clearance: LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check. Reporting Requirements Section 101212: Changes at the facility must be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation. 12 Noon, Incidental Medical Services ( IMS): This facility does not plan to provide Incidental Medical Services \342\200\223 IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. Page 5 LPA observed next to preschool class room 202 is one bathroom with 3 toilets & 3 sinks and next to room 203 another bathroom with 3 toilets & 2 sinks. In room 303 is a bathroom with 3 toilets and 2 sinks. In room 101 is 1 toilet and 2 sinks. In room 102 is a bathroom with 1 toilet and 2 sinks. The toddler class room #103 there is 1 toilet (15 Capacity) and 2 (30) sinks. Total Preschool toilets 11(165) and sinks total 11. (165 Capacity) LPA advised the Applicant to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov . LPA observe the parent board located in the preschool office to have all the required licensing forms accessible for parents to view, which included the Parent\342\200\231s Rights Poster (with complaint hot line). An approved fire clearance was granted on 07/12/2021 for capacity of 169 (15 Toddlers and 154 Preschool) 1 PM LPA also consulted with the applicant regarding COVID-19 health and safety guidelines on this date. Licensing staff observed COVID- 19 postings posted in different areas of the day care on this date. During this inspection LPA also discussed PIN 20-06 CCP, Social and Physical Distancing Guidance and Healthy Practices for Child Care Facilities in Response to The Global Corona virus (COVID-19) Pandemic Written in Collaboration with The California Department of Education. LPA reviewed with the applicant the Child Care Covid-19 Self-Assessment guide that the applicant will fill out and email to the LPA by 06/02/2021. The following correction need to be made prior to submitting application for final approval. - Photos of all Documents posted and class rooms completely set up. -Final calculations of the indoor measurement for capacity determination. -Final File review. The application will be submitted to be licensed after completion of correction needed . Once licensed, the applicant is required to adhere to the terms and limitations stated on the license 1: 30 PM. An exit interview was conducted with Heather Warma, Administrator & Lydia Guerrero, Director., and a copy of this report has been signed by and given as well as Appeal Rights, given and explained on this date
