What the inspector wrote

On 11/18/24 Licensing Program Analyst (LPAs) Mary Silva and Kamile Martin conducted an unannounced Annual/Random inspection to the above facility. Upon arrival licensee Rukshani Sunnadeniyage greeted licensing staff and guided LPA(s) on a tour of the facility. The reason for the inspection was explained. LPA(s) provided licensee with a copy of the Entrance Check List. Licensing staff observed eight (9) children in care at the time of inspection, three (3) were infants. Per licensee nine (9) children are currently enrolled. Facilities hours of operation are Mon-Fri from 6:30am-5:30pm. This is a single-story home which consists of 4 bedrooms, 2 bathrooms, living room/daycare room, dining room, kitchen, attached garage, front, back yard and side yard. Children have access to the living room/daycare room, 1 bathroom in the hallway, 1 bedroom, which is used for napping, dining room, and the back yard. Children do not have access to the kitchen, 3 bedrooms, 1 bathroom, front yard, side yards and attached garage. Currently residing in the home are licensee, licensee’s spouse and two minor children. During the inspection, assistant and spouse were present. During the tour Licensing staff observed cleaning compounds under the sink of the bathroom and kitchen. Licensee removed the items during the inspection and understood that all poisons must be locked and made inaccessible to children. Licensing staff observed the following required posted documentation in the main entry way of the facility: Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. Licensing staff reviewed facility records for LIC 9040- Facility Roster. Licensing staff observe a record of the last drill conducted on 11/11/2024. ________________Page 1 of 4_____________ The home was observed to be clean and orderly, with central air and heating. Licensing staff observed the smoke detector and the carbon monoxide detector to be in operable condition. Licensing staff observed the valve on the required 2A 10BC fire extinguisher indicates fully charged with a service tag dated 02/24/2024. Reminded licensee fire extinguisher should be serviced yearly. The home maintains telephone service via land line. There are toys and other age-appropriate material available for children. Licensee indicated meals are prepared. There are some children that bring food from home. There are no children with food allergies enrolled at this time. Isolation area for ill children to rest is located in the dinning room. The children use the backyard for outdoor play. The outdoor play areas were observed to be fenced. Licensing staff observed a changing table under a shadowed patio cover where children in diapers are changed. A trash can was observed to be next to the changing table. Licensing staff observed the outdoor back yard to have a play structure, toys and other materials for children. Licensee states there are no bodies of water on the premises and LPA did not observe any bodies of water during this visit. Child’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, and LIC 995A Notification of Parents’ Rights. Licensee file was reviewed for the following: LIC 508- Criminal Record Statement, Proof of immunizations against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse. Pediatric First Aid and CPR certification on expires 11/09/2024 for licensee, and on 07/20/2026 for assistant. Mandated Reporter Training has been completed and expires on 08/08/25 for licensee and on 08/17/2024 for assistant. www.mandatedreporterca.com ____________________________Page 2 of 4__________________________ Licensing staff discussed the safe sleep regulations and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. Licensing staff also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee currently has (3)infants enrolled. Incidental Medical Services (IMS) policy was discussed. Assistant # 1 indicated no children are utilizing incidental Medical Services. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. MyChildCarePlan.org – Centers and Family Child Care Homes - Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. Megan’s Law - Family Child Care Homes - During the exit interview, Licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. _____________________________Page 3 of 4____________________ Based on Licensing staff observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal rights were given.
