What the inspector wrote

At 8:20 am Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced annual random inspection. LPA met with Facility Representative Director Elizabeth Ramirez, who guided the LPA on a tour of the facility. This is a preschool age program licensed for 28 children which operates Monday – Friday from 7:00 AM – 5:30 PM. Per the Director 28 children are currently enrolled. A children’s roster was available for review. Licensee, Leticia Olague arrived around 9:00 am. All areas identified on the Facility Sketch were inspected. The following staff was present during this inspection: Room # 1: Staff # 1, and # 2 with 2 children, Room # 2 Staff # 3 with 10 children and Room # 3 Staff # 4 with 5 children. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision, including visual supervision, of a teacher at all times. The licensed facility is within the conditions, limitations, and capacity specified on the license. LPA observed the facility to be clean, sanitary and in good repair. Furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. LPA did observed 1 urinal to be out of service- Per Director a work order has been placed. The facility still has enough toilets for their capacity. All materials and surfaces accessible to children are toxic free. -------------------------------------------------------- pg.1 of 4 -------------------------------------------------------- At this time, the office is used as an isolation area. There are chairs and cot available if needed. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted. Facility offers am and pm snack- parents provide lunch. All meals are provided by parents and placed in an insulated lunch bag and hung on a rack. There is drinking water available in all indoor classrooms and water bottles are taken outdoors. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions and other items that are dangerous to children are stored in an area inaccessible to children. LPA observed sleeping arrangements in form of cots. Per Director sheets are washed every Friday by the facility. Smoke and Carbon Monoxide detectors were observed and are operable. At 9:15 am LPA observed that the last disaster drill conducted and documented was on 3/14/25. Outdoor playground is located on the back of the facility. Teachers and children go through the church parking lot to get to the playground. The equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Children’s Records were reviewed and were observed to be complete. ------------------------------------------------------- pg. 2 of 4 ------------------------------------------------------- At 9:30 am Staff records were reviewed – LPA observed that 3 out 4 staff present did not have a current AB1207 Mandated Child Abuse Reporting certificate on file. On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed. Incidental Medical Services (IMS) policy was discussed. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, ---------------------------------------------------------- pg. 3 of 4 --------------------------------------------------------- must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Deficiencies that are being cited need to be cleared to protect the children’s health and safety. A notice of site visit was given and must remain posted for 30 days.
