What the inspector wrote

On 04/28/2025 Licensing Program Analysts (LPA) Mary Silva conducted an unannounced POC (Plan of Correction) inspection to insured that the Type B deficiencies cited on 03/21/2025 have been cleared. LPA met with Director, Yolanda Bush who guided analyst on a tour of the facility. There were 7 children present in the Discovery Room with 1 teacher during this inspection. No other children were present today. The following was observed: - During file review immunization: MMR and TDAP were missing from staff #2. LPA advised the licensee how to access forms, regulations and quarterly updates on the Child Care Licensing Website at: www.ccld.ca.gov Based on the LPA observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children\342\200\231s health & safety. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Director, Yolanda Bush.

Staff 2 did not have verification of TDAP and MMR, which poses/posed a potential health, safety or personal rights risk to persons in care.
