What the inspector wrote

All staff interviewed reported that children’s medications are stored in a locked closet within the classroom. Each child’s medication is kept in their individual bags, labeled with their name and accompanied by all required documentation. Staff confirmed that the medications were labeled appropriately. LPA interviewed parents of children enrolled in the infant program. No disclosures were made to support the allegation above. During the course of the investigation, LPA conducted a full inspection of medication storage in the infant program and observed two children with medication enrolled at the facility. Upon review, one child’s medication was found not to be in its original packaging as required by Title 22 regulations. The label had been altered and did not match the original prescription instructions. The facility has been cited for this deficiency during its annual inspection on May 2, 2025. Based on interviews, observations and disclosures made during the department investigation, the preponderance of evidence and the standard has been met, therefore, the allegation is found to be substantiated. These incidents pose a potential risk to the Health and Safety of the children in care. However, no deficiency will be cited at this time, as the issue was previously cited during the facility’s annual visit on May 2, 2025. A notice of site visit was given and must remain posted for 30 days. Appeal Rights provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal rights were given and explained. Exit interview conducted, report provided to facility Assistant Director, Stephanie Rodriguez.
