What the inspector wrote

Annual Inspection was conducted in Armenian. On 01/08/2026, at 1:00pm, Licensing Program Analyst (LPA) Shushanik Safaryan conducted an annual random inspection to the above facility. LPA met with Talin Aleksandy , Licensee to whom the reason for the visit was explained. Per Licensee, there are 7 children currently enrolled. There were 7 children and licensee`s assistant (mother) present upon arrival. The licensee was observed to be operating within the license capacity limitations. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report. Per licensee operation hours are Monday to Friday, 8:00 a.m. to 5:30 p.m. During this visit, all areas identified that are accessible for children to use were inspected for safety, comfort, and cleanliness. This is a single-story home that consists of 3 bedrooms, 1 and \302\275 bathrooms, living room, dining room, kitchen, patio, front yard, back yard (fenced) and basement. Per licensee, parents enter the home through the main entrance which leads to the living room. Page 1 of 5 Areas accessible to children are living room (main daycare area), dining room, bathroom in the hallway, last bedroom, half bathroom in the bedroom, patio, and backyard (fenced). Per licensee areas off limits for children and parents include: 2 bedrooms, kitchen, basement and front yard. **Rooms that are off-limits need to be made inaccessible during operating hours** At 1:00 pm , Licensee guided LPA to the tour of the home. LPA was toured starting from the living room where LPA observed 6 children preparing to nap. In the living room, LPA observed children\342\200\231s toys, cubbies with their names .LPA observed storage located in the hallway had a child safety knob . Next LPA toured the dining room and observed child safety gate separating dining room from living room main day care area. Per licensee, children pass through the dining room when they need to exit to the patio or go to the bathroom. In the dining room, LPA observed the dining table and chairs. LPA observed another child safety gate separating kitchen area from the dining room. LPA observed off limit bedrooms doors have child safety knobs on the doors. Next LPA toured the bathroom in the hallway and did not observe any hazards. LPA observed personal care items stored on the high shelf in the shower area, which were inaccessible to the children. LPA toured the bedroom in the hallway and observed play pin and adult bed. LPA observed one child in the play pin. Per licensee, bedroom only used for nap time for this child per mother`s request and play pin was provided by child`s parent. Next LPA toured the \302\275 bathroom, per licensee \302\275 bathroom not used for children and door is always locked. Facility License, Emergency Disaster Plan, Parent\342\200\231s Rights Poster, and Earthquake Preparedness Checklist were observed to be posted in the hallway area next to the main entrance. A current children\342\200\231s roster was available for review. There is telephone service via landline and cellphone that stays at the facility during operation hours. Detergents, cleaning compounds, and medications were observed to be inaccessible to children. Per Licensee, there are no poisons in the home. LPA advised that any poison should be locked under key or combination lock. Licensee states that there are no firearms or weapons stored in the home. Page 2 of 5 The valve on the required 2A 10BC fire extinguisher indicates fully charged and LPA observed receipt with a service date 03/2025. Smoke alarm was observed in the hallway. Per licensee it contains carbon monoxide devices too. LPA was unable to test since children were napping and it was too high and had paint over the description. LPA discussed with the licensee need of carbon monoxide devise in the facility. All homes shall conduct fire and disaster drills at least once every six months and document the date and time of each drill. Last drill documented was conducted on 10/2025. The home is observed to be clean and orderly. There are first aid supplies available. There are age-appropriate toys available for children. Per licensee, she provides food for children in care. The Licensee was advised that if food is brought from the children\342\200\231s homes, all containers must be labeled with child\342\200\231s name and properly stored or refrigerated. Currently, children are using the patio and backyard for outdoor play time. Children use the door from dining room to exit to the patio area .LPA observed child safety gate at the end of the patio area and beginning of stairs. Licensee state supervision is always provided and children under supervision use stairs to go down to the concrete area. The outdoor play area was observed to be fenced and gated. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA observed storage shed that was locked .LPA did not observe any objects that can pose danger to children. No bodies of water were observed on this day. Per licensee, no pets on the premises. Licensee records were reviewed. The licensee has completed training in preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expire on 03/2026. LPA observed that the Licensee does have proof of Mandated Reporter AB 1207 compliant Child Care Training Certificate on file with expiration on 4/07/26. www.mandatedreporterca.com Page 3 of 5 Children\342\200\231s records were reviewed, including emergency information. During the review LPA observed one 4 children present had incomplete forms. LPA discussed with the licensee regarding corrections. Per licensee, she will complete and send it to LPA. LPA created LIC 857, Children\342\200\231s Record Review, which documents children\342\200\231s files reviewed during this inspection. Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home. LPA did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs, and/or any other item that fall into these categories are not permitted in a family childcare facility. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Licensee states she is currently caring for one infant over 12 months , LPA observed 15-minute checks. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at Page 4 of 5 https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LPA advised the licensee how to access forms, regulations and quarterly updates on the Child Care Licensing website at www.ccld.ca.gov . The Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee Talin Aleksandy, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Based on the LPA\342\200\231s observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children\342\200\231s health & safety. LPA discussed all the deficiency cited during today\342\200\231s inspection with the licensee. Per licensee, she understands all the deficiencies and plans of correction.
