What the inspector wrote

Licensing Program Analysts, (LPAs) Staicy Perry and Diana Ortiz conducted an unannounced Case Management Inspection. LPAs met with Ashley Garcia, Associate Teacher and Johnathan Rodriguez Instructional Assistant. There were 8 infants and 2 staff present. The purpose of this visit is to follow up on (2) incidences reported to the Department on 4/4/2025 & 4/11/2025. During course of the inspection, LPAs were taken on a tour of the facility. All teacher/child ratios were met and criminal record clearance for staff was verified. Shortly after Vanessa Guzman Education Specialist arrived to assist licensing since both staff present were in ratio and unable to assist. The department received an incident report from this facility on April 4, 2025, for an incident that occurred at the facility on April 3, 2025 indicating that Early Head Start class at Bella Vista was cancelled due to not having enough staff. Additionally, the department received an incident report from this facility on April 11, 2025 for an incident that occurred at the facility on April 11, 2025 indicating that Early Head Start class at Bella Vista was cancelled due to not having enough staff. It was reported to the department in a timely manner. During the inspection LPAs conducted (2) staff interview regarding the incidences. LPAs obtained documentation in the form of declaration, children\342\200\231s roster staff file review conducted and Personnel roster were obtained. According to S#1, Facility was closed on April 3, 2025 due to one of the lead teachers being absent and teacher substitute was not available. Facility was also closed on April 11, 2025, due to all 3 teachers were absent and qualified teachers were not available. LPAs advised S#1 about Personnel requirements in Title 22 regulations. S#1 understood and filled out a declaration of their plan on ensuring ratios are maintained and per S#1 staff are to call the Head Start and early Learning center programs office for an administrator or an education specialist can support in an event that there is an emergency. Based on observations and information obtained during this investigation and records reviewed, no deficiencies will be cited. Facility is currently in compliance with Title 22 Regulations. Appeal rights discussed and provided. Exit interview conducted with Vanessa Guzman.
