What the inspector wrote

On 4/16/25 at 9:45am, Licensing Program Analyst (LPA), Stephanie Li conducted an unannounced Case Management inspection for an increase in capacity. Upon arrival, LPA met with Director Gitty Rivkin. Applicant is requesting to be licensed for 16 preschoolers ages 3-5 in classroom #3. Hours of operation will be Monday Friday 8:00am- 4:30pm . Present during today’s inspection were 0 children with 1 staff. (School was on spring break) Appropriate ratios and supervision were observed. All indoor space utilized for the children was inspected today. LPA informed Gitty Rivkin that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. LPA continued to tour the facility and measured new additional indoor and outdoor activity space. Total indoor activity space measured for sufficient space to accommodate the requested capacity of (16) children. Classroom 3 and central room was measured. LPA observed classroom 3 to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping mats, bookshelves, and other activity supplies for the children. Central room is going to be used as the sick area. Drinking water is available in the classrooms via bottled water. LPA observed a hot water boiler in the sanctuary near the kitchen. Sanctuary is accessible from classroom 3. Hot water boiler was observed to be moved into the off limits kitchen during the visit. LPA observed a door leading out to busy street on Walnut street. Bell chime alert must be placed on door as door is accessible to children in classroom 3. There are no bodies of water or weapons on the property. Fire clearance was granted on 3/6/2025. LPA observed a total of 2 sinks and 2 toilets available for children’s use. These are sufficient to accommodate the requested capacity of 16 children. LPA observed step stools placed in front of toilets during visit. Page 1 of 2 The facility currently has a fully fenced playground area. Fencing wrought iron is at least four feet high. The previous measured outdoor space is 3139.10 sq ft. which is sufficient to accommodate the requested capacity. Shade is provided via shade awnings. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is a climbing play structure on the playground for children ages (Little Tykes: no age manual) which is properly anchored. There is adequate cushioning in fall zones of climber provided by foam padding. LPA observed slide and ladder within 3 feet. Director will place foam bumper on slide edge to provide cushioning. Drinking water is available via water bottles. LPA observed all hazardous items on the playground to be inaccessible to children. Director was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). The following corrections are needed prior to the issuance of the license: 1. Safety bell/chime on door at the back of the sanctuary leading to Walnut street. 2. Cushioning/bumper on slide adjacent to climbing ladder Director Gitty Rivkin understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. Per Director, she will submit corrections by the end of April. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
