Case Management inspection for CAPACITY INCREASE conducted by Licensing Program Analyst (LPA) Jennifer Hua. LPA met with licensee Elizabeth Gonzalez. The purpose of the visit was announced. Covid-19 risk assessment was conducted. LPA observed 4 children in care. Licensee's spouse was also present. Per licensee, operation hours are M-F 7am - 7pm. Per licensee there are a total 4 adults and 1 minor reside in the main house. There is 1 adult resides in the back house. Licensee took LPA on a tour of the facility. Upon arrival LPA observed 3 children in care supervised by licensee, during the visit, 1 child arrived. LPA observed 2 dogs in the backyard in the off limits area. Licensee stated, the big dog belongs to her and dog sitting the medium size dog. Landlord Notification/Consent on file.. This is a single story home with 3 bedrooms, 2 bathrooms, 2 living rooms, family room, dinning area, kitchen, fenced backyard and back house that is attached to the main house. Licensee states that there is no access to the back house from inside of the main house. The back house has a living room/dinning room, kitchen, 1 bathroom and 1 upstairs bedroom based on Pre-licensing visit. Off Limits: Front living room, 3 bedrooms, 1 bathroom, rear yard, swimming pool, back house. Day care areas: Kitchen, family room, back living room, dinning area, bathroom located on the right side of the hallway and rear side yard. LPA observed there is a swimming pool on premise. LPA observed there is iron fencing is 5 feet tall, the gate self closes, self latches, opens away from the pool and ensured that there is nothing obstructing the view of the pool. LPA also observe the block wall next to the iron fence measured at 4'10" tall. Licensee stated will add additional fencing on top of the block wall to ensure it is at least 5 feet high. LPA reminded licensee that any climbing objects (chairs, bench. ladders) must be away from pool fencing at all times. Per licensee, there are no weapon on the premise. There are age appropriate toys, equipment were observed for children. Licensee stated will throw away the saucer chair that was observed in the back living room. Licensee stated it is not being used. Posting requirements were observed to be posted at the time of inspection. Children Forms/Records to Keep in Your Family Child Care Home were reviewed. LPA advised the licensee how to access forms, regulations and quarterly updates on line at: www.ccld.ca.gov LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Licensee's adult daughter who is home from college since last Sunday night does not have fingerprint clearance on file. The licensee has proof of current pediatric first aid and CPR (expires: 9/10/2024). Licensee has completed required mandated reporter training on 4/17/2023. Roster reviewed. Per licensee, drills were conducted 2/11/23 as observed on the drill log. LPA observed fire extinguisher size (2A10BC) is mounted on the family room wall, it is fully charged and was purchased on 9/2022 according to licensee. No smoking, No infant walkers, No Johnny jumpers, No saucer chairs, No trampolines and any other item that falls into this category are not permitted in a family child care facility. INFANT CARE: The infant will sleep in the day care where they are constantly supervised. Appropriate sleeping arrangements and cribs will be available/observed. (one crib for each infant in care is required). Cribs or play yard will not hinder the entrance or exit from the sleeping space, mattresses shall be firm and covered with a fitted sheet that overlaps the underside so it cannot be dislodged. Cribs and play yards shall be free of loose articles and objects. No objects shall be hanging above or attached to the side of the crib. LPA did not observe any infants swaddled while in care. LPA advised the Licensee that infants shall be placed on their backs for sleeping and shall be supervised. Infants shall be checked on every 15 minutes and document the time of each 15-minute LIC 9227 Plan shall be completed for each infant up to 12 months of age. LPA provided the licensee with a copy of A Child Care Provider’s Guide to Safe Sleep, by American Academy of Pediatrics. LPA also consulted and explained Child Abuse Reporting, Never Shake a Baby, and Safe Sleeping practices. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. Deficiencies cited on attached 809D Upon receipt of this report documenting a Type A deficiency, the licensee shall do the following: 1. Post the Notice of Site visit and any licensing report documenting a Type “A” deficiency. 2. The report and the Notice of Site visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. 3. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). 4. The Acknowledgement form (LIC 9224) must be maintained in each child’s file immediately upon receipt from parent. A copy of the parent Acknowledgement of Receipt of Licensing Reports Form was provided during this visit. Fire clearance has been approved. Once deficiencies are corrected, the capacity increase shall be approved. Exit interview was conducted with Elizabeth Gonzalez, licensee, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role. .
