What the inspector wrote

Licensing Program Analyst (LPA) Nolan Tcheng conducted an unannounced Case Management-Incident inspection for the purpose of following up on an incident reported to the Department on 04/24/2024. Upon arrival at 1:35pm, LPA met with Program Manager Patricia Yaghnam, to whom the purpose of the inspection was explained. Due to staff absence on 05/03/2024, LPA is conducting a subsequent inspection on this date. Tour of the facility was provided. There were children present during the time of inspection. Census was taken. There were 42 children with 11 staff members. Staff #3 (S3) interview was conducted during today's inspection. Per S3, they were having dance time with the class around the small play house in the play yard. Child #1 (C1) attempted to climb a small fence on the play house, S3 intervened and redirected child. S3 continued to lead the class around the play house during their dance time. C3 was in line dancing and then left the line and climbed the small bench attached to the play house and fell off. S3 states they did not have enough time to stop and redirect the child before it happened. Pictures of the area were obtained. All equipment is age appropriate and child's access to the areas involved was quick and unavoidable. Per administration, camera footage was not available due to the cameras around the play area, not directed at the play area involved in the incident. Based on interviews and observation, licensing staff has determined facility staff could not have done anything to prevent the incident from occurring. The incident is considered an unusual incident. There were no deficiencies observed in regards to today's visit. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Program Manager Patricia Yaghnam, at 3:15pm. Copy of Report provided. END OF REPORT
