What the inspector wrote

On 04/16/2025, at 2:30 pm Licensing Program Analyst (LPA) Shushanik Safaryan conducted an unannounced Case Management Inspection to follow up on the incident that took place on 03/25/2025. This is an infant program with toddler options. Upon arrival ,LPA met Laura Hernandez, and Patricia Yaghnam, Managers of Early Care and Education who guided LPA on a tour of the facility. Census was taken. There were 39 children and 12 staff members. On 03/25/2025, an unusual incident report was submitted to the Department regarding child sustaining an injury on the facility grounds .The Unusual Incident/ Injury Report was reported as required. During the visit, LPA inspected the area where alleged incident took place and did not observe any hazards.( Pictures were taken). Based on the information obtained there were 6 children present with 2 staff members during the incident . LPA conducted an interview with the Staff member and Program director . Per staff member and Program director first aid was provided ,parents were notified, and ouch report was created which was obtained during this inspection. Per staff member , child was on tricycle on the bike path and was using his feet to move forward, when fell and hit his head. Per staff member, she observed the incident , ran to the child and applied ice pack. Information was obtained that child was not wearing helmet during the incident .Program Director stated per meeting that was conducted after helmets are mandatory when children are using bikes. Based on the information obtained ,no citations were issued at this time .Exit interview conducted with Patricia Yaghnam, Manager of Program and Copy of Report and Notice of Site visit was provided . The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. End of the report .
