On 11/18/2025, Licensing Program Analyst(LPA) Shushanik Safaryan conducted an unannounced Case Management inspection to the above facility to ensure Type B deficiencies cited on 11/05/2025 have been cleared. Upon arrival, LPA met with Facility Representative Suzie Johnson, to whom the reason for the visit was explained . Tour was provided . During the tour , LPA observed 4 infants with one staff member and assistant director. Next LPA observed 9 toddlers with 3 staff members. During this visit, LPA obtained meeting agenda conducted on 11/10/2025 with all staff members. LPA reviewed meeting agenda regarding ratio , supervision of infants during the nap time, food service and hazards. Per Facility Representative staff member will be scheduled to stay in the nap room with the sleeping infants. During the visit, LPA cleared deficiency cited on 11/05/2025 and provided a copy of the Licensing Report to Licensee and issued POC clearance letter. The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Page 1 of 1
