PRELICENSING INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an announced pre-licensing follow up inspection to the above facility on 02/24/2023. LPA arrived at the facility at 10:35 AM and met with Ani Akopian, Applicant who guided analyst on a tour of the facility. Per applicant operation hours will be Monday to Saturday, 7:00 a.m. to 10:00 p.m.. Applicant states she will care for children 0-12 years old. Applicant was previously licensed at 550 W. Harvard Street, Glendale, CA 91204 with facility number 198020487. Fire clearance for large family child care was granted on 12/05/2022. All areas identified on the facility sketch were inspected. This is a two story home located on the first level. The home consists of 4 bedrooms, 2 restrooms, living room, dining room, kitchen, front yard, back yard fenced, detached guest house in the backyard under this address which consists of a room, one bathroom and a kitchen, detached converted garage (storage). Per applicant, parents will enter the facility mainly through the side gate which leads to the backyard and daycare area or through the main entrance which leads to the living room. There is detached dwelling in the backyard with separated address, Detached converted second story portion of detached garage into an ADU with 1915 Glenwood RD unit A, Glendale , CA 91201 address. A copy of Certificate of Occupancy on file. Areas that are accessible to children are as follows: Bathroom in the hallway, living room, dining room, two bedrooms in the first floor, and backyard (fenced). Areas off limits based on facility sketch submitted to children and parents include: Kitchen, master bedroom and bathroom in the master bedroom, one bedroom in the second floor, detached converted garage ( storage) in the backyard, detached guest house in the backyard, and front yard. **Rooms that are off-limits need to be made inaccessible during operating hours** REPORT CONTINUES ON NEXT PAGE 1 of 2 The following corrections were observed during this inspection: 1. LPA observed 2A 10 BC fire extinguisher was placed in a cabinet in the kitchen. It was purchased on 12/16/22 as read by LPA from the receipt. 2. Applicant submitted a copy of Preventative Health and Safety training certificate, completion date: 12/17/22. 3. Applicant submitted an updated facility sketch. 4. LPA observed a safety gate was installed to the kitchen to make the kitchen inaccessible to the children. 5. Applicant added a plastic door knob and a chain lock on the main entrance for additional safety. 6. Applicant stated detached guests house in the back yard is under this address. Applicant submitted copy of TB test clearances for Ovanes Avanesian and Sona Avanesian. 7. Applicant submitted an updated application form LIC 279). 8. Applicant added a gate in the living to the stairs which leads to the second floor off-limit bedroom. A large family child care license will be granted upon receipt of proof of corrections for the above. Once licensed, the applicant is required to adhere to the terms and limitations stated on the license. The applicant’s signature on this report acknowledges that they have signed the Application for a Family Child Care Home License (LIC 279) under the penalty of perjury that the statements on the application and any attachments are correct.
