What the inspector wrote

PLAN OF CORRECTION INSPECTION WAS CONDUCTED IN ARMENIAN Licensing Program Analyst Anomeh Eivazian conducted an unannounced Plan of Correction (POC) inspection to the above facility on 03/06/26 to ensure Type B deficiencies cited on 02/06/26 have been cleared. LPA arrived at 12:00 PM and met with Karmen Shakhbazyan and Ani Tokhunts, licensee’s assistants. Upon LPA’s arrival to the facility licensee, Ani Akopian was not present in the home. At 12:10 PM, licensee arrived at the facility who stated she had stepped out to go to her daughter’s school. At 12:15 PM, licensee, Ani Akopian guided analyst on a tour of the facility. During this inspection there were 11 children present in the facility, three being infants with licensee and her two assistants. The following has been observed: - LPA observed licensee completed infant nap chart for currently enrolled three infants. - During this inspection LPA observed an infant was napping and napping bedroom door was open and an assistant was supervising the napping infant. - Per licensee, child#1 is not attending at this time and LPA observed child#4’s immunization record in the file. - Per licensee, currently she has 13 children enrolled in her daycare. LPA reviewed current facility roster, and it was completed. - Licensee's assistants, Karmen Shakhbazyan and Ani Tokhunts have TB test clearances and proof of immunization against measles, missing proof of immunization against pertussis (TDAP) and Influenza. Per licensee, she does not have access to detached garage. Per licensee, she verified with landlord, that unit A tenants have access to the detached garage and tenants in the unit A also completed criminal record clearance and have clearance in Licensing Information System. REPORT CONTINUES ON NEXT PAGE 1 of 2 An updated facility sketch was submitted during this inspection. At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today from the areas that were inspected today. LPA advised the licensee how to access forms, regulations and quarterly updated on the Child Care Licensing Website at: www.ccld.ca.gov . LPA cleared deficiencies on this date and provided a copy of the Licensing Report to Ani Akopian, Licensee, and issued POC clearance letter. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee, Ani Akopian. REPORT END 2 OF 2
