What the inspector wrote

On 10/31/2024 at 9:30 am, Licensing Program Analysts (LPAs), Crystal Green and Saul Valenzuela conducted an unannounced Case Management inspection for an increase in capacity. LPAs met with Facility Representative Cynthia Davila and Rajeev Nandakumaran. This facility is currently licensed as an infant program with a toddler program with a capacity of 40 children: (16) infant children from ages 0 to 24 months in classroom FL 101 and FL 102 and (24) toddler children from ages 18 to 36 months in classroom FL 103 and HH 101. The Licensee, Bridge Education, has requested to increase the infant/toddler program to 76 children: (40) infant children from ages 0 to 24 months in classroom FL 101 and FL 102 and (36) toddler children from ages 18 to 36 months in FL 103 and HH101. Also operating on the premises is a preschool program (198021231) licensed for a capacity of 160 children from ages 2 to 6 years old (9) classrooms which will HH 105, HH 106, HH 107, HH 108, HH 109, OH 103, OH 104, OH 105, and MS 110. This facility is located on the premises of Lake Avenue Congregational Church. Parents will utilize the main parking lot for parking which is accessible from Maple Street or Villa Street. Parent will enter the facility via the Family Life entrance. Children will be escorted by their parents to their designated classroom. There will be a parent board with the required postings located within each classroom. Hours of operation will be Monday to Friday from 7:30 am to 5:30 pm. Present during today\342\200\231s inspection were 14 infant children with 4 staff and 15 toddler children with 3 staff. Appropriate ratios and supervision were observed. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Facility Representative that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. Page 2: LPA continued to tour the facility and measured all indoor and outdoor activity space. Total infant indoor activity space measured 1426.84 square footage, which is sufficient to accommodate the requested capacity of (40) infant Children. Total toddler indoor activity space measured 1905.14 square footage, which is sufficient to accommodate the requested capacity of (36) toddler Children. LPA observed all indoor activity space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, 8 cribs in FL 101, 6 cribs in FL 102, napping cots/mats for infants who can climb out of a crib, bookshelves, and other activity supplies for the children. Toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water is available in the classrooms via filter water jugs along with children\342\200\231s personal drinking bottle. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 10/03/2024. High chairs and feeding tables have broad-based legs, plastic seats are in good repair and trays lock onto chairs. High chairs are made of washable, moisture-resistant material. Changing tables have at least 1\342\200\235 padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3\342\200\235 high. LPAs observed the changing table to be a fixed built-in structure located on the opposite side of the sink. Diapering sink is not used for meal preparation or dishwashing. Cribs meet regulatory requirements. Crib area was enclosed by a shelf about 2 feet high with a 3 feet high gate divider. Sleep logs are kept in within the Bright Wheel app. Needs and Services Plans and Sleep Plans are kept in within the Bright Wheel app. There is at least one potty chair or toilet for every five infants. LPA observed a total of (7) sinks available for children\342\200\231s use. These are sufficient to accommodate the requested capacity of (76) children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the HH104. The facility currently has a fully fenced playground area which will be redesignated for the infant program. Fencing is made of a concrete wall and is at least four feet high. The total square footage for the infant outdoor activity space is 3326.90 square footage, which can accommodate 44 children at one time. Per the facility representative, the infant children will access their outdoor play yard via the hallway and through the main office (HH104) and the toddler program will share the yard using a staggered play schedule. Page 3: A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure. Shade is provided via shade sail. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is adequate cushioning in fall zones of climber provided by outdoor cushions mats. Drinking water is available via each child\342\200\231s personal water bottle. LPA observed all hazardous items on the playground to be inaccessible to children. Applicant was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. The following corrections are needed prior to granting the capacity increase request. -Licensee must obtain 4-foot-high soundproof barrier for the crib area located within FL 101 and FL 102. -Licensee must obtain an approval for outdoor waiver for the toddler program. -Licensee must obtain an approval waiver regarding the changing tables located the infant classrooms. Once the corrections are received and waiver request approved, the facility will be licensed to care for (76) children: (40) infants ages 0 to 24 months in classrooms FL 101 and FL 102, (36) toddlers ages 18 to 36 months in classrooms FL 103 and HH 101. Exit interview conducted and report was reviewed with the Licensee Representatives, Cynthia Davila and Rajeev Nandakumaran. A notice of site visit was given to Licensee Representatives and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
