What Happened
Complaint investigation substantiated two Type A violations: unlicensed staff providing supervision without criminal record clearance, and inadequate supervision resulting in multiple child injuries including a concussion. Single staff member worked at facility; unlicensed individuals (A1, A2, A3) provided supervision during operator vacations.
Observations
- A1 provided supervision to 5 children without additional staff, lacking criminal record clearance
- A2 and A3 provided supervision when licensed operators on vacation; A3 covered breaks during nap time
- Child #1 sustained concussion on 08/12/2025 and three additional injuries within same month, all caused by other children
- Four incident reports documented Child #1 injuries from other children within one month
- Only one staff member (S1) available for supervision and director duties
- S1 reported multiple times to facility representatives that additional supervision help was needed
- Sign-in/out sheets and staff timecards not available during investigation
- Staff records not maintained at facility
