What the inspector wrote

An announced pre-licensing site inspection was conducted today by Licensing Program Analyst (LPA) Crystal Green to inspect and measure facility for capacity determination. Licensing staff met with Director, JoAnn Cendejas. Licensee is requesting a change of location for their existing preschool program (198004595) from 631 E. Saxon, San Gabriel, CA 91776. This facility located on the premises of St. Anthony Church and St. Anthony School which is a private elementary. Licensing staff was guided through a tour of the proposed preschool classrooms. This facility will have (2) preschool classrooms; Room P1 and Room P2. The preschool classrooms were observed to have its own separate entrance. Per Director, parents will arrive through the gates located on California Ave. and escort their child directly to the preschool classroom to sign in. Sign in and sign out will be conducted at classroom P1. LPA observed classrooms P1 and P2 to have individual storage space was observed available for each child within the classrooms. Toys were observed to be clean and safe. Furniture and equipment were inspected for age appropriateness and good repair. LPA observed the children to have access to 2 toilets and 2 sinks located within classroom P1. All toilets and sinks are in safe and sanitary operating conditions. LPA advised the director that children from classroom P2 must be supervised at all times when transiting to utilize the restroom in classroom P1. The Director informed that she understands. Water will be accessible to children via water dispenser with disposable cups and the children will also have their individual sippy cup. The facility will have cots available for children. LPAs observed the preschool to have emergency/first aid kits located throughout the facility. There are also smoke detectors and fire extinguishers located throughout the facility. The clinic office located within the main office will be used as the designated ill isolation room. Ill children will utilize the clinic restroom. Page 1of 3 LPA tour the outdoor space located next to the preschool classrooms. LPA observed the outdoor area to be designated for the elementary school children. Per the Director, the applicant is requesting a waiver to share usage of the elementary school playground using a staggered play schedule. LPA observed the playground to have a paved bike path with a climbing play structure. The playground area was observed to have adequate shading provided via canopies. All areas around or under high climbing equipment, and slides were observed to have wood chips cushion material to absorb a fall. The children will have water accessible outdoors via their personal sippy cup. This facility will also offer a lunch program to the children in care, or the parents will provide prepared snacks and lunch for the child. Food brought into the classroom from the lunch program will already be prepared. No transportation will be provided by the facility. Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. This facility plans to provide Incidental Medical Services \342\200\223 IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Fire Clearance has been received by the Department. Based on measurements obtained today, capacity for the preschool program will be thirty (30) children. LPA discussed with the facility representative that an outdoor waiver request must be approved prior to allowing the children to share the elementary play yard on a staggered schedule. Per the Director, the children will use the play yard located at the existing location which was observed to be within walking distance from classroom P1 and P2 until the waiver is approved. LPA obtained pictures of the outdoor play area during this inspection. Exit interview was conducted with Director, JoAnn Cendenjas and a signed copy of this report was provided. Page 3 of 3
