What the inspector wrote

On 02/01/2024 at 10:00 am, Licensing Program Analyst (LPA), Crystal Green conducted an announced Pre-Licensing inspection for a new license. Upon arrival, LPA met with Applicant Representative Victoria Zargaryan. Applicant, Dream Preschool and Academy, has requested to be licensed for 60 school age children from ages 5 to 17 years old in 5 classrooms located on the second floor of the building. Hours of operation will be Monday to Friday from 12 pm to 7pm. This facility is in a two-story building. The entrance to the facility will be accessible from Kentworth St., parents will enter the parking lot from California Ave. and exit onto Kentworth St. Per Applicant, staff will be located in the parking lot to supervise drop offs and pickups. There is also a preschool program (198021113) located on the same premise that operators out of the first floor only. The school age program will operate on the second floor of the building. LPA observed the second floor to be accessible via 2 stairwells located within the building. LPA observed both stairwells entry points to have a child safety gate located at the bottom of the stairs to prevent access to children under the age of 5. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Victoria Zargaryan that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. A fully equipped first aid kit is located in the Directors office and upstairs classrooms. There is an operational carbon monoxide detector on site located throughout the premises. All required licensing documents were observed posted in the main hallway outside of the Directors office. Children will be signed in and out near the main entrance of the facility using the Brightwheel app. LPA continued to tour the facility and measured all indoor and outdoor activity space. This school-age program will operate from a total of 5 classrooms located on the second floor. Total indoor activity space measured 2132.80 square footage, which is sufficient to accommodate the requested capacity of 60 Children. Page 2: LPA observed all indoor activity space to be complete with safe age-appropriate furniture and equipment, including tables, chairs, cubbies, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via bottled water. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 10/05/2023 by the Glendale Fire Prevention Department. LPA observed a total of 4 sinks, 4 toilets, and 2 urinals available for children\342\200\231s use. These are sufficient to accommodate the requested capacity of 60 children. Restrooms afford privacy for children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be located in the main office. There is a fully functioning kitchen located on the premises which will be used to prepare food on site. The kitchen is located on the first floor which will be maintained inaccessible to the children via door that only staff will be allowed to have access to. The kitchen area currently includes refrigerator, sink, and stove, microwave, food prep countertop. The kitchen area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. This facility has 2 fully fenced playground area (Playground 1 and Playground 2) that is currently designated for preschool program. Shade is provided via canopy. There are sufficient outdoor age-appropriate toys and play equipment available on both playground area. There is a climbing structure located on Playground 2 for children ages 2 to 6 which is properly anchored. There is adequate cushioning in fall zones of climber provided by outdoor playground mats. Playground 1 measured for 2,712.53 square footage and Playground 2 measured for 5521.70 square footage. Per Victoria Zargaryan, the school-age program will utilize Playground 2. Playground 2 can accommodate 60 children at a time. A playground waiver to share Playground 2 at different times in the PM due to the school-age program not having a separate designated outdoor space shall be submitted and approved prior to licensure. Drinking water will be available via bottled water. LPA observed all hazardous items on the playground to be inaccessible to children. Applicant Representative was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. Transportation will be provided to the school- age children. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Page 3: Applicant representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. No corrections are needed to the facility at this time. A license for 60 children will be granted upon a final file review and approval of the outdoor waiver request.
