What Happened

Child sustained hand injury in incident due to inadequate supervision - staff supervising 11 children while changing another child in restroom. Type A citation issued for immediate risk.

Observations

  • On 07/08/2024, Child #1 sustained hand injury when restroom door closed while child was exiting
  • Staff #1 was inside restroom with 11 children while changing another child on changing table near sink
  • Inadequate supervision: One qualified staff person cannot properly supervise 12 children (11 remaining + 1 being changed) while changing a child in the restroom
  • Normally another staff member would assist but none available at time of incident
