What the inspector wrote

On 08/12/2024 at 10:00 am, Licensing Program Analyst (LPA), Crystal Green conducted an announced Pre-Licensing inspection for a new license. Upon arrival, LPA met with Applicant Representatives, Talin Elmedijan and Alin Matossian. The Applicant, The Busy Mind, LLC., has requested to be licensed for 30 preschoolers ages 2 to 5 years old in classroom 1 and classroom 2. Hours of operation will be Monday to Friday from 8:30 am to 4:00 pm. All indoor and outdoor activity space utilized for the children was inspected today. LPA observed this facility to have 2 floors. The first floor contains classroom 1. The main office is located on the second floor along with classroom 2. LPA informed the applicant representatives that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Alin Matossian stated that they will be stored in a locked cabinet located within the office. A fully equipped first aid kit is located throughout the facility. There is an operational carbon monoxide detector on site located throughout the facility. All required licensing documents were observed posted next to the main entrance to the facility. Children will be signed in and out outside at the main entrance. LPA continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space measured 1129.10 square footage, which can accommodate the requested capacity of 30 children. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water will be available in the classrooms via disposable water bottles and the children\342\200\231s personal water bottles. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 4/25/2024 by the Los Angeles County Fire Department. Report Continues Page 1 of 3 LPA observed a total of 2 sinks and 2 toilets available for children\342\200\231s use. These are sufficient to accommodate the requested capacity of 30 children. There is a separate staff restroom equipped with a toilet and a sink located on the second floor. The isolation area for children who are ill will be the staff restroom. Facility will provide AM/PM snacks. Parents will be providing their child\342\200\231s lunch. There is a food prep kitchen located on the second floor will not be accessible to the children in care. The kitchen area currently includes mini refrigerator, sink, and food prep countertop. The kitchen area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. The facility currently has a fully fenced playground area. Fencing was observed to be at least four feet high. LPA obtained measurements of the designated area which calculated to be 1428.00 square footage, which can only accommodate 19 children at one time, insufficient for the number of children being requested. A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure. There are sufficient outdoor age-appropriate toys and play equipment available throughout the main playground area. Shade is provided via umbrellas and tree shade. There is sufficient cushioning in fall zones of the playground. Drinking water will be available via children\342\200\231s personal water bottles. LPA observed all hazardous items on the playground to be inaccessible to children. Applicant Representatives was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA advised applicant to keep the document on file regarding completing the lead testing. Applicant representatives was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Applicant Representatives was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communications. During this inspection, LPA obtained a waiver request letter and pictures of the outdoor play area. The Applicant Representatives, Talin Elmedijan and Alin Matossian, was informed that the waiver requests will be reviewed by the Regional Office for final approval in order to obtain the capacity of 30 preschool children. No corrections are needed. A license for 30 preschool children from ages 2 to 5 in classroom 1 and classroom 2 will be granted upon a final file review and approval of the outdoor waiver request. Exit interview conducted and report was reviewed with the Applicant Representatives, Talin Elmedijan and Alin Matossian.
