What the inspector wrote

Licensing Program Analyst (LPA) Stephanie Li conducted an unannounced Case Management inspection due to an incident that occurred on 9/18/2025 and was reported to the Department on 9/18/2025. A risk assessment was conducted. LPA met with assistant director Lisa Cheyno. Reason for the visit was explained. There were 55 children and 19 staff. On 9/18/2025, an unusual incident report was made to the Department regarding an incident where infant child had a fever and seizure. Facility had contacted parents to inform them of the fever and per their sick policy, the child needed to be picked up immediately. Child began seizing and brought to assistant director. 911 and parents were called immediately. Both arrived within 10 minutes. Paramedics took child\342\200\231s vitals and ruled seizure as febrile seizure and had staff undress infant. Paramedics did not take the child to the hospital. Father took child to the pediatrician. During todays visit, LPA interviewed assistant director regarding the incident, reviewed child\342\200\231s file and health history, reviewed sick policy and protocols, obtained copies of LIC624 and follow up notes from parents regarding pediatrician visit. Per assistant director, teacher was transitioning child from her nap to bring to designated isolation area when seizure occurred. Per parents follow up message, no medical diagnosis was given. Pediatrician ran several test and child tested positive for Covid. Child has returned to school with no follow up required by doctor. pg 1 of 2 LPA advised facility to track and document seizure incidents in children\342\200\231s records for teacher access and future monitoring of re-occurrence of febrile seizures when child is ill. Per assistant director, the Brella app allows for allergy tagging for teachers to view and she will add seizure tag to the Brella app. Per assistant director, they also use the Procare app and she will add seizure incidents/notes to the child\342\200\231s profile so the information is visible to all staff. LPA advised to review any emergency care (seizure) protocols regularly with all staff when any child enrolled in facility experiences emergency care. Per assistant director, staff meeting is scheduled for first week of November to review emergency care protocols with all staff. Based on information obtained during this investigation, no follow up is necessary regarding the incident reported. No deficiencies are being cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
