What the inspector wrote

Licensing Program Analyst (LPA) Stephanie Li conducted an unannounced Case Management inspection due to an incident that occurred on 10/27/2025 and was reported to the Department on 10/27/2025. A risk assessment was conducted. LPA met with assistant director Lisa Cheyno. Reason for the visit was explained. There were 53 children and 17 staff. On 10/27/2025, an unusual incident report was made to the Department regarding an incident where a child bumped his mouth on a shelf, chipping his tooth and lip was bleeding. Staff administered first aid and sent incident report to parents. Facility reported the incident to licensing and LPA. Parents observed the incident and were not too concerned about the chip. During todays visit, LPA interviewed assistant director regarding the incident, inspected the classroom furniture, reviewed child\342\200\231s info, and obtained copy of photo taken of injury. Child is a toddler DOB: 5/20/2024. He is 17 months old and in the Peach toddler room (ages 12-18months). Per Director, child is very active and was running full force when he collided into the toy shelf. Per director, child runs head first into things and is very active. Staff has rearranged furniture and centers in classroom to illuminate collisions. Staff has designated entrance of room (open area) as the action area. LPA observed soft materials, paddings, cushions, and rugs in the action area. page 1 of 2 Based on information obtained during this visit, no follow up is necessary regarding the incident reported. No deficiencies are being cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with assistant director Lisa Cheyno. page 2 of 2
