What the inspector wrote

Licensing Program Analyst (LPA) Stephanie Li conducted an unannounced Case Management inspection due to an incident that occurred on 3/17/26 and was reported to the Department via phone call on 3/17/26. A risk assessment was conducted. LPA met with Assistant Director Lisa Cheyno, to whom the reason for the visit was explained. Director guided LPA on a tour of the facility. There were 53 children present and 20 staff. The facility was observed to be operating within the license capacity limitations. On 3/17/2026, an unusual incident report was made to the Department regarding an incident where a child left the group and hid by the water fountain in the hallway during a transition from the play yard back to the classroom. Child entered the bathroom for several minutes, exited and began walking back to class. A resource teacher came out of the staff room and discovered the child in the hallway and walked him back to class and notified the classroom teacher. During todays visit, LPA interviewed assistant director Lisa. Lisa reviewed video surveillance of child and reviewed Procare name to face log. Lisa discovered name to face check in on Procare was not conducted by teachers upon returning to the classroom. Lisa had provided follow up documents during a previous visit. Documents obtained include: copy of LIC624, LIC500 personnel report, LIC9040 children’s roster, staff schedules, training review with staff, and staff write up. Staff that did not complete name to face headcount voluntarily is no longer working at facility. Per director, prior to incident admin was checking name to face log on a weekly basis. Now they are checking name to face log on a daily basis. Director completed group and one on one training to reinforce name to face headcount logging before and after every transition. There is always admin staff stationed at the front desk at the entry to facility which is 10 feet away from the water fountain and bathroom area child was hiding in. Page 1 of 2 Based on information obtained during this investigation, a deficiency is being issued per Title 22 regulations, see page LIC809D. Deficiencies issued need to be cleared to protect the health and safety of children in care. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Assistant Director Lisa Cheyno. Page 2 of 2

Supervision shall include visual observation.This requirement was not met evidenced by: Based on interview and record review, the licensee did not comply with the section cited above as child walked away from teachers unnoticed during transition and was unsupervised for some minutes before being returned to the classroom, which poses/posed a potential health, safety or personal rights risk to persons in care.
